Tiivistelmä
Opinnäytetyöni aiheena on alkoholivieroitushoidon toteutuminen vuodeosastolla. Tilaajana toimii Uudenkaupungin Lähisairaalan osastot 1 ja 2. Aihe työlleni valikoitui omasta kiinnostuksesta ja tilaajan esille tuomasta tarpeesta.
Työn tarkoituksena on laatia alkoholivieroitushoito-opas vuodeosastojen hoitajien työvälineeksi, sekä osaksi osastojen perehdytystä. Työn tavoitteena on lisätä tietoa hoitajille alkoholivieroitushoidosta ja sen onnistumisen tärkeydestä. Lisäksi henkilökohtaisena tavoitteenani on oppia lisää aiheesta ja saada projektin kautta uusia työkaluja omaan työhöni.
Ennen oppaan laadintaa selvitin hoitajien kokemuksia alkoholivieroitushoitopotilaan kohtaamisesta. Selvitin myös heidän tietotaitoaan potilasryhmän lääkkeellisestä hoidosta ja jatkohoitoon ohjaamisesta pienimuotoisen kartoituksen avulla.
Kartoitukseen vastasi molemmilta osastoilta sairaanhoitajia sekä lähi- ja perushoitajia. Vastaaminen oli täysin vapaaehtoista ja tapahtui työajalla anonyymisti. Vastauslomakkeet palautettiin palautuslaatikkoon ja tein niistä yhteenvedon.
Vastauksissa esille nousi hoitajien turhautuminen alkoholiriippuvaisen hoitoon, etenkin silloin, kun potilas ei itse ymmärrä omaa riippuvuuttaan tai sen vakavuutta. Lääkehoito oli suurimmalla osalla vastanneista hyvin halussa ja he tiedostivat sen tärkeyden. Jatkohoidon tärkeydestä moni vastaaja oli hyvin perillä, tiesi miten ja mitä kautta potilas jatkohoitoon ohjataan. Moni kuitenkin pohti jatkohoidon tarpeellisuutta ja hyödyllisyyttä siinä tilanteessa, kun potilas ei omaa riippuvuuttaan tiedosta. Näiden esille tulleiden asioiden pohjalta laadin alkoholivieroitushoito-oppaan. Oppaan sisältö pohjautuu tutkittuun teoriatietoon.
Projekti eteni suunnitellun aikataulun mukaan. Projektini oli vaiheistettu järkevästi ja eteni loogisesti. Asettamani tavoitteet toteutuivat hyvin ja olen tyytyväinen tulokseen. Sain rakentavaa palautetta ohjaavalta opettajaltani ja tilaajaorganisaation edustajalta projektini eri vaiheissa. Muutin opasta ehdotusten perusteella ja mielestäni siitä tuli hyvä ja selkeä. Se on laajuudeltaan kolmetoistasivuinen ja käsittelee alkoholiriippuvuutta sairautena, sekä päihderiippuvaisen kohtaamista hoitajan näkökulmasta. Pääpaino on kuitenkin alkoholivieroitushoidossa ja mitä eri asioita siihen liittyy. Tilaajaorganisaation edustaja antoi oppaasta positiivista palautetta. Projektin aikana jäin pohtimaan, että jatkossa joku opiskelija voisi tehdä tutkimuksen alkoholivieroitushoitopotilaiden kokemuksista hoitoonsa liittyen. Tutkimuksen kautta saisi käsityksen siitä, miten he hoitonsa kokevat. The topic of my bachelor’s thesis is the materializing of alcohol rehabilitation in the hospital ward. The thesis was ordered by Uusikaupunki Health Center’s ward 1 and 2. I chose this topic because of my own interest and a need showed up by the orderer.
The objective of the thesis was to draw up a guide to alcohol rehabilitation for nurses who work at hospital wards so they can use it at their work and for orientation of new workers at wards. The target is to make nurses more conscious about alcohol rehabilitation and how important it is to succeed. Also, my personal goal was to learn more about this topic and find some new methods to my own work through this project.
Before drawing up the guide I found out nurses’ experiments of encountering an alcohol rehabilitation patient. I also figured out their knowledge of patients’ medical nursing and guiding them to follow-up treatment by doing a small survey.
The survey was answered by both nurses and practical nurses. Answering was totally based on voluntariness and happened at working time anonymously. Answering sheets were returned to a return box. I made a conclusion about the answers.
It was seen in the answers that the nurses were frustrated at nursing the alcohol dependence patients, especially when the patient doesn’t understand their own dependence or it’s gravity themselves. Most of the respondents mastered the medical treatment well and they were conscious of its importance. Many of the respondents also had a good knowledge about follow-up treatments and they knew how and trough what the patient will be referred to follow-up treatment. However, many of them were considering the necessity and usefulness of the treatment when the patient isn’t conscious of their own dependence. I drew up the guide to alcohol rehabilitation based on these findings. The content of the guide is based on researched, theoretical information.
This project proceeded as the schedule was planned. It was phased sensibly and proceeded logically. The targets I made materialized well and I am pleased about the result. I got constructive feedback from my supervisor teacher and the representative of the orderer at different stages of my project. I developed the guide by their suggestions and in my opinion, it became good and clear. It has 13 pages and deals with alcohol dependence as a sickness and encountering the alcohol dependence patient from nurse’s point of view. However, the weight is put on alcohol rehabilitation and what kind of subjects are related to it. The representative of the orderer organisation gave positive feedback about the guide. During this project I considered that maybe a follow-up research could be a study about the alcohol rehabilitation patient’s experiences about their treatment. That would bring information about how they experience their treatment themselves.
