Tiivistelmä
Tulsa Pro -hoito on magneettiohjattu ultraäänihoitolaite eturauhasen syövän ja hyvänlaatuisen liikakasvun hoitoon. Turun yliopistollisessa keskussairaalassa (Tyks) otettiin syksyllä 2017 käyttöön Suomen ensimmäinen Tulsa Pro -hoitolaitteisto.
Tämä kehittämisprojekti on osa Tyksin Tulsa Pro -hanketta. Tulsa Pro -hoitojen tarve on kasvussa ja hoitojen määrää on tarkoitus lisätä. Kehittämisprojektin tarkoituksena oli tunnistaa Tulsa Pro hoidon prosessista hoitoja hidastavia tekijöitä ja laatia Tulsa Pro -hoidosta prosessikuvaus. Kehittämisprojektin aikana tutkittiin Tulsa Pro -hoitojen aikana täytettyjä hoitopäivälokeja. Kehittämisprojektin tavoitteena oli kehittää Tyksin Tulsa Pro -hoitojen toimintamallia, jotta pystytään vastamaan hoitojen lisääntyneeseen tarpeeseen ilman, että potilasturvallisuus vaarantuu tai hoitojen tulokset heikkenevät.
Hoitopäivälokeista kävi ilmi, että hoitojen kokonaiskesto on keskimäärin 5 tuntia. Hoitojen aikana ilmenneitä poikkeavuuksia olivat esimerkiksi ultraääniapplikaattorin liikuttelu ja ultraääniapplikaattorin tekniset ongelmat sekä ilmakuplien poistaminen peräsuolesta. Poikkeamat, joiden selvittelyyn kului paljon aikaa, olivat esimerkiksi asemointijärjestelmän ongelmat, odottaminen ja ilmakuplien poistaminen peräsuolesta. Kehittämisprojektin myötä voidaan todeta, että yksittäinen poikkeama ei hidasta hoitoa kovinkaan paljon, mutta jos yhden hoidon aikana ilmenee useampi poikkeama, voivat ne viivästyttää hoitoa jopa tunteja.
Kehittämisprojektin myötä Tulsa Pro -toimintamalliin luotiin kehittämisehdotuksia. Hoitojen aikana pitää tarkkaan miettiä, ovatko kaikki toiminnot tarpeellisia. Teknisten ongelmien suhteen voidaan laitevalmistajalle antaa enenevissä määrin palautetta ja kehittämisehdotuksia. Eri tahojen välisellä yhteistyöllä on mahdollista kehittää Tulsa Pro - hoidon toimintamallia entistä sujuvammaksi. Tulsa Pro is MRI-guided ultrasound treatment for prostate cancer and benign prostate hyperplasia. The first Tulsa Pro treatment of Finland was introduced in Turku University Hospital (Tyks) in autumn 2017.
This development project was part of Tyks´ Tulsa Pro -project. The demand for Tulsa Pro treatments is growing and the number of treatments is meant to be increased. The purpose of the development project was to find delays from the Tulsa Pro treatment process and write a process description. During the Tulsa Pro treatments, treatment day logs were filled. The research part of the development project was based on the logs. The aim of the development project was to develop a Tulsa Pro operation model to respond to the increased demand of treatments without compromising patient safety or impairing treatment outcomes.
According to the treatment logs one Tulsa Pro treatment takes about 5 hours. The deviations during the treatments were for example moving of the ultrasound applicator, technical problems with the ultrasound applicator and removal of air bubbles from rectum. The deviations that took the most time were technical problems of the positioning system, waiting and removal of air bubbles from rectum. Based on the development project it can be stated that one deviation does not slow down the treatment that much, but if there are many deviations during the treatment, it may result in a delay of even several hours.
During the development project, development proposals for the Tulsa Pro operation model were created. During the treatments it is very important to consider, whether every function is really needed. Regarding the technical issues, more feedback and development proposals should be given to the equipment manufacturer. The cooperation of the stakeholders enables the further development of the Tulsa Pro operation model.
