Tiivistelmä
Eturauhassyöpä on yleisin miesten syöpä, jonka hoito suunnitellaan potilaskohtaisesti. Eturauhassyövän hoitomuotoja ovat leikkaus-, säde- ja hormonihoito tai näiden yhdistelmä. Tässä opinnäytetyössä keskitytään sädehoitoon ja erityisesti potilaan asettelun toistettavuuteen. Opinnäytetyön tarkoituksena on selvittää kuvantaohjatusti sädehoidettujen eturauhassyöpäpotilaiden asettelun toistettavuutta suunnittelukuvauksen ja hoidon välillä. Tavoitteena on kehittää hoidon laatua ja turvallisuutta.
Käytetty aineisto kerättiin sädehoidon hoidonvarmistusjärjestelmään tallentuneista potilaskohtaisista siirtoja kuvaavista tiedoista. Potilassiirtojen suuruutta suhteessa sisäisiin markkereihin tarkasteltiin suunnittelukuvauksen osalta sekä potilaan viiden ensimmäisen hoitokerran osalta korkeus (vrt)-, pituus (lng)- ja poikittaissuunnassa (lat) sekä hoitopöydän kierron asteena (rot). Kaikkien suuntien siirroista (cm) laskettiin minimi- ja maksimiarvot, mediaani, keskiarvot ja keskihajonnat. Aineisto analysoitiin kuvailevan tilastotieteen menetelmillä.
Tulosten perusteella pääasiassa suurimmat siirrot tapahtuivat korkeussuunnassa, toiseksi suurimmat pituussuunnassa ja pienimmät siirrot tapahtuivat poikittaissuunnassa. Rotaatiossa tapahtui suuria siirtoja, mutta ne sijoittuivat pääasiassa ensimmäiselle hoitokerralle, jonka jälkeen siirrot olivat pienempiä.
Tutkimuksen tulosten perusteella voidaan päätellä, että huolimatta tarkasta asettelusta ulkoisten ihomerkkien avulla, tapahtuu potilaskohtaisia hoitopöydän siirtoja jokaisella hoitokerralla. Sädehoidon kuvantaohjaus osoittautuu tarpeelliseksi ja tärkeäksi menetelmäksi hoidon kohdistamisen ja sen osuvuuden varmistamisessa. Ulkoisten ihomerkkien käyttö ei siis yksinään takaa asettelun toistettavuutta. Mahdollisia keinoja asettelun toistettavuuden parantamiseksi voisivat olla hoitopöytään kiinnitettävät fiksaatiot, joiden avulla potilas asettuisi samaan kohtaan hoitopöydällä jokaisella hoitokerralla.
Jatkotutkimusaiheena voisi tutkia eturauhassyöpäpotilaiden kuvantaohjatun sädehoidon potilaskohtaisten hoitopöydän siirtojen suuruutta ilman asettelussa käytettäviä ulkoisia ihomerkkejä. Asettelun apuna voisi sen sijaan käyttää hoitopöytään kiinnitettäviä fiksaatioita. Näitä tuloksia voisi vertailla tässä työssä saatuihin tuloksiin. Prostate cancer is the most common cancer in men and the treatment of the patient is planned individually. The different forms of treatment of prostate cancer are surgery, radiotherapy, hormonetherapy or the combination of them. The focus in this thesis is on radiotherapy and especially on the repeatability of positioning the patient. The purpose of this thesis is to review the repeatability of positioning between planning imaging and radiotherapy treatment of patients treated with image guided radiotherapy of prostate cancer. The goal of this thesis is to develop the quality and safety of treatment.
Research material was collected from the treatment assurance system of the radiotherapy outpatient clinic. The radiotherapy table movements were compared to fiducial markers inside patient’s prostate in three different directions: vertical, longitunal and lateral. Table rotation was also measured. Minimum, maximum, median, mean and standard deviation was calculated from every table movement in every direction (centimeter, cm). The material of the thesis was analysed with descriptive statistical methods.
According to the results the maximum values of movements were in vertical direction. Second-largest values of movements were in longitude direction and the smallest were in lateral direction. Table rotation was largests in the first treatment. After the first treatment rotation was fairly low.
On the basis of this study it can be deduced that despite careful patient positioning with skin marks table movements are needed in every treatment. Image guided radiotherapy proves to be essential in targeting and ensuring treatment into the right place. It can also be deduced that skin marks do not guarantee repeatability of patient positioning. Possible other ways to enhance the repeatability of patient positioning could be fixations that are attached to the treatment table. These can help the patient settle in the same place on the table in every treatment.
Further research could study image guided radiotherapy of prostate cancer and measure the table movements of patients that do not have skin marks. Fixations attached to the treatment table could be used in positioning. The results could be compared to the results of this thesis.
