Tiivistelmä
Opinnäytetyön tarkoituksena oli kartoittaa potilaan kotiuttamisen tarkistuslistan vaadittavat tekijät Turun yliopistollisen keskussairaalan (TYKS) neurokirurgian osaston hoitohenkilökunnalle, kirurgisen aivokasvainpotilaan jatkohoidon järjestämisen ja kotiuttamisen tueksi. Opinnäytetyön tavoite on tukea ja kehittää kirurgisten aivokasvainpotilaiden hoidon jatkuvuutta erikoissairaanhoidosta kotiutuessa. Työn pohjalta kehitettyä tarkistuslistaa voidaan käyttää neurokirurgianosaston kotiuttamistilanteissa.
Tutkimusmenetelmänä opinnäytetyössä oli narratiivinen kirjallisuuskatsaus. Käytössä oleva aineisto haettiin eri tietokannoista (Cinah, JBI, Medic, PubMed ja Terveysportti). Lopulliseen kirjallisuuskatsaukseen valittiin 15 tutkimusta.
Tutkimuksen tulosten mukaan potilaiden kotiutuksessa käytettävän tarkistuslistan tulisi sisältää potilaanlääkitys,jatkohoito-ohjaus, yhteydenotto sekä kuljetus. Lääkityksen tulisi sisältää potilaan lääkelista, reseptit, mahdolliset lääkemuutokset ja lääkeneuvonta. Jatkohoito-ohjaukseen kuuluu haavanhoito, mahdolliset apuvälineet ja tarvittava informaatio potilaalle. Yhteydenotto koskee omaisten, kotihoidon ja kotiutustiimin yhteydenpidon. Kuljetus kattaa potilaan jatkohoitoon siirtymisen.
Opinnäytetyön tuloksista voidaan päätellä, että kotiutusprosessissa on runsaasti paranneltavaa. Usein hoitotyössä esiintyy inhimillisiä virheitä sekä hoitotapahtumia jää tekemättä. Kotiutuksen tarkistuslistan käyttö olisikin kehotettavaa. Kotiutusta tulisikin tutkia jatkossa laajemmin. Mikäli saataisiin riittävästi näyttöä ja tuloksia kotiutuslistan käytöstä, voitaisiin se ottaa yleiseksi käytännöksi ja täten yhtenäistää sairaaloiden toimintaa. he purpose of this bachelor’s thesis was to find out the required factors for a checklist that is used for surgical brain tumor patients discharge. It was made for the nursing staff of the Department of Neurosurgery of Turku University Central Hospital (TYKS) to support the followup care and discharging of surgical brain tumor patients. The aim of the thesis is to develop and support the continuity of care for surgical brain tumor patients who are discharged from specialized medical care. The checklist developed by the thesis can be used in discharge situations of the neurosurgery department.
The research method used in the thesis was a narrative literature review. The material used was retrieved from various databases (Cinah, JBI, Medic, PubMed and Terveysportti). For the final literature review 15 studies were selected. According to the results of the study, the checklist used in patient discharging should include medication, follow-up care, contact and transportation. Medication should include the patient’s medication list, prescriptions, possible medication changes and medication advice. Follow-up care includes wound care, possible aids, and the necessary information for the patient. The contact concerns the communication between relatives, home care or a home care team. Transportation covers the patient’s transition to follow-up care.
The study concludes that there is much room for improvement in the discharge process. Human errors often occur and treatment events remain undone. The use of a discharge checklist would therefore be encouraged. Discharging should be studied more extensively in the future. If sufficient evidence and results were obtained on the use of a discharge checklist, it could be standardized as a general practice in the future and unify hospital proceedings.
