Tiivistelmä
Kliiniset laboratoriotutkimukset ovat tärkeä osa kliinistä päätöksentekoa, jossa luotettavat tutkimustulokset ovat potilasturvallisuuden edellytys. Tykslab tuottaa kustannustehokkaasti korkealaatuisia ja potilaslähtöisiä laboratoriopalveluita. Akkreditoitu näytteenottopalvelu edellyttää henkilöstön pätevyyttä suoriutua näytteenottotyöstä laatusuositusten mukaisesti. Laboratoriotutkimusprosessin preanalyyttinen vaihe on todettu riskialtteimmaksi vaiheeksi poikkeamien synnylle. Ammattitaitoinen näytteenotto edellyttääkin mm. näytteenoton teoriaosaamista, vakioitujen toimintaohjeiden tuntemusta ja tietojen soveltamisosaamista. Näytteenoton teknisen suorituksen lisäksi toiminnan vaikutuksien ymmärtäminen ja poikkeamien hallinta ovat osa osaamisvaatimuksia.
Tykslabissa oli tunnistettu tarve tuottaa näytteenoton osaamisen hallinnan ja johtamisen malli näytteenottotyöhön tuleville muille kuin bioanalyytikon koulutuksen saaneille terveydenhuollon ammattilaisille, kuten lähihoitajille ja sairaanhoitajille. Kehittämisprojekti eteni osaamisen hallinnan perusprosessia mukaillen. Projektin menetelmät olivat kirjallisuuskatsaus, puolistrukturoitu kysely yksilöhaastatteluin (n=6) sekä toiminnallinen kehittämistutkimus, jossa hyödynnettiin projektiorganisaation dialogista keskustelua ja tiedon reflektiota.
Näytteenoton osaamisen hallinnan ja johtamisen malliksi muodostui näytteenoton perehdytyskokonaisuus, johon tuotettiin Moodle® oppimisympäristöön näytteenoton perehdytysmateriaali ja teoriakoe sekä Tykslabin laatujärjestelmän mukaiset lomakkeet näytteenoton näyttökokeeseen ja lupaan itsenäiseen työskentelyyn. Kokonaisuus kokoaa yhteen projektissa tuotetut tuotokset ja Tykslabin näytteenoton perehdytyksen ja osaamisen arvioinnin välineet.
Näytteenoton perehdytyskokonaisuuden avulla Tykslab voi luotettavasti perehdyttää kohderyhmän näytteenottotyöhön ja arvioida näytteenoton osaamista. Odotettavissa on lisäarvoa tuottavia vaikutuksia asiakkaiden, henkilöstön, prosessien ja taloudellisesta näkökulmasta. Kokonaisuuden avulla tavoitteena on vastata Tykslabissa näytteenottotyön osaamistarpeeseen nyt ja tulevaisuudessa. Clinical laboratory tests are an important part of clinical decision making. Reliable research results are a prerequisite for patient safety. Tykslab produces cost-effective, high-quality, and patient-centered laboratory services. A well accredited laboratory sampling service requires well qualified personnel to perform the sampling work in accordance with the quality recommendations. The preanalytical phase of the laboratory research process has been identified as the most risky phase for the development of irregularities. Professional sample collecting requires e.g. theoretical knowledge of laboratory sampling,
knowledge of standard operating procedures and ability to apply these information’s. In addition to the technical performance of sampling, understanding the effects of operations and managing deviations are part of one’s competence requirements.
Tykslab had identified the need to produce a model for sampling competence management for healthcare professionals, other than a biomedical laboratory scientist trained for sampling work, such as community practical nurses. The development project proceeded in accordance with the basic process of competence management. The project methods were a literature review, a semi-structured questionnaire with interviews (n = 6), and dialogue between the project organization and information reflection.
The project produced a sampling orientation model, sampling orientation material and a theory test for the Moodle® E-learning environment. In addition, Tykslab's quality system sampling demonstration test form and a permit form for independent work was created. The model brings together outputs and Tykslab's sampling orientation and competence assessment tools.
With the help of the sampling orientation model, Tykslab can reliably familiarize the target group with the sampling work and assess the sampling competence. Positive feedback is expected from the customer, personnel, internal process, and the finance perspective. The goal is to meet the skills needed for sampling work at Tykslab now and in the future.
