Tiivistelmä
Alkoholin riskikäytön tunnistaminen ja varhaisen tuen antaminen kuuluvat terveydenhuollon tehtäviin. Lyhytneuvonnan avulla potilasta voidaan tukea kun alkoholin käyttö on runsasta, mutta siitä ei ole vielä koitunut merkittäviä haittoja. Riskikäyttäjien määrän on arvioitu Suomessa olevan jopa puoli miljoonaa. Potilaan alkoholinkäytön puheeksiottaminen on perusteltua sekä sairauksien ehkäisyn että hoidon vuoksi. Tiedot alkoholin käytön haitoista velvoittavat terveydenhuoltoa puuttumaan asiaan.
Kyseessä on työelämälähtöinen kehittämisprojektina toteutettu opinnäytetyö. Kehittämisprojekti toteutettiin yhteistyössä Eurajoen perusterveydenhuollon kanssa ja projekti eteni konstruktivistisen mallin vaiheiden mukaisesti. Yhtenäisten toimintaohjeiden puuttuessa alkoholin puheeksioton toteutuminen on hyvin vaihtelevaa. Projektiorganisaatio koostui Eurajoen perusterveydenhuollon lääkärien, sairaanhoitajien ja terveydenhoitajien vastaanotosta sekä päivystyksestä, kotisairaanhoidosta ja terveyskeskussairaalan osastosta.
Kehittämisprojektin tavoitteena on selkeyttää ja lisätä alkoholin mini-interventioiden toteuttamista luodun mallin avulla ja kehittää ehkäisevää päihdetyötä Eurajoen perusterveydenhuollossa. Kehittämisprojektin tuotoksena syntyi alkoholin mini-intervention toimintamalli. Toimintamalli kehitettiin kuusihenkisen ammattilaisista koostuvan projektiryhmän avulla. Toimintamallin kehittämisessä hyödynnettiin myös aiempia tutkimuksia ja kirjallisuutta.
Toimintamallissa on kuvattuna alkoholin mini-intervention prosessi: missä tilanteissa alkoholinkäyttö otetaan puheeksi, alkoholinkäytön puheeksioton tukena käytettävä mittari, jatko-ohjauksen ja kirjaamisohjeet. Toimintamallissa käytetään AUDIT-C mittaria. Lääkärit käyttävät vastaanotolla puheeksioton tukena ja seulontavälineenä potilaiden itse täyttämää terveyssuunnitelmaa. Kotisairaanhoidossa, terveyskeskuksen sairaalaosastolla ja terveydenhoitajien vastaanotolla alkoholinkäytöstä kysytään kaikilta asiakkailta.
Seulontarajojen ylittyessä tulee toimintamallin mukaisesti interventio antaa RAAMIT -malli huomioiden. Intervention tukena henkilökunta voi käyttää jo olemassa olevaa materiaalia. Mikäli asiakas/potilas haluaa apua tai tukea alkoholinkäytön vähentämisessä, tulee hänelle jatkohoito mahdollistaa ja oikea jatkohoitotaho ohjata. Tärkeintä on kuunnella asiakkaan/potilaan omaa tahtoa ja motivaatiota. Recognizing risk consumption of alcohol and providing early support is part of primary health care’s responsibilities. Through guidance, the patient can receive support when the use of alcohol is significant but has not yet caused significant damages. It has been estimated that in Finland there could be nearly half a million risk users of alcohol. Bringing up the patient’s use of alcohol is justifiable from the point of preventing illnesses and providing treatment. The information regarding the harms of alcohol use requires healthcare personnel to act against it.
The operations model is a thesis created as a work-based development project. the development project was created in cooperation with the primary healthcare of Eurajoki and the project advanced according to the constructive model’s stages. Without common outlines, bringing up the topic of alcohol consumption has not been consistent. The organization for the project was made up of receptions of doctors, nurses, and public health nurses as well as from the ER, district nursing, and the department of health centre hospital.
The development project intends to clarify and increase the use of mini interventions with the help of the created model and develop preventative alcohol and drug use at the primary health care of Eurajoki. Through the development project, the operations model for mini interventions regarding alcohol usage was created. The operations model was developed with a project group consisting of six professionals. Earlier research and literature were used in advantage when creating the operations model.
The process of mini interventions has been depicted in the operations model: in situations where the consumption of alcohol is brought up, there are measures on how to bring up the topic of alcohol consumption, guides on after-counseling, and guides on making entries. In the operations model, the AUDIT-C measure is being used. The doctors when bringing up the topic of alcohol consumption and as a screening method, turn to the health plan filled by the patients themselves. In district nursing, the health centre hospital, and at the reception of public health nurses all the patients are asked of their alcohol usage.
Once the screening limits are exceeded, the intervention of the project model must be given taking the RAAMIT-model into account. The staff can use already existing materials as support for the intervention. If the patient/client wants help or support in decreasing alcohol consumption, must further treatment be made possible and guided for proper further treatment. Most important is to listen to the patients/clients own will and motivation.
