Tiivistelmä
Kriittisesti sairastuneen potilaan hoitolaitossiirto on virheille altis vaihe potilaan hoidossa. Näiden hoitolaitossiirtojen voidaan olettaa lisääntyvän lähivuosina merkittävästi vaativan päivystyksellisen hoidon keskittyessä suuriin yksiköihin. Potilasturvallisuuteen on viime vuosina keskitytty entistä enemmän niin ensihoidossa, kuin muissakin terveydenhuollon toimintaympäristöissä. Yhtenäiset ja sujuvat toimintatavat ovat avainasemassa hoidon laadun sekä turvallisuuden kannalta.
Opinnäytetyön tavoitteena oli osaltaan edistää potilasturvallisuuden kulttuuria organisaatiossa sekä luoda toimeksiantajan toiveita vastaava tarkistuslista. Opinnäytetyön toimeksiantajana toimii Ensihoito 9Lives Oy. Kirjallisuuskatsauksessa käydään läpi kriittisesti sairastuneen potilaan tyypillisimpiä hengityksen, verenkierron ja tajunnan ongelmia sekä perehdytään lyhyesti niiden hoitomuotoihin.
Tämän toiminnallisen opinnäytetyön tarkoituksena oli tuottaa tarkistuslista kriittisesti sairastuneen potilaan hoitolaitossiirrosta kuvailevan kirjallisuuskatsauksen pohjalta. Aihe rajattiin koskemaan ainoastaan aikuispotilaiden hoitoa, mutta tarkistuslista pyrittiin pitämään soveltuvana useaan eri erikoisalaan. Kirjallisuuskatsaus luotiin käyttämällä viimeisintä saatavilla olevaa tutkittua tietoa aiheesta, hyödyntäen eri tietokantoja sekä alan oppikirjoja. Opinnäytetyöhön sisällytettiin myös perusteluja tarkistuslistojen merkittävästä hyödystä potilasturvallisuuden kannalta. Jatkossa olisi kannattavaa tutkia tarkistuslistan vaikuttavuutta hoitolaitossiirtojen turvallisuuteen sekä pohtia tämän opinnäytetyön lopputuotteena syntyneen tarkistuslistan kehitystarpeita. An inter-hospital transfer is a phase in the treatment of a critically ill patient, which is disposed to error. In the near future the number of these transfers is expected to increase, when advanced emergency care is centralized into big departments. The focus on patient safety has been progressing during the past years, in prehospital emergency care as in other healthcare environments. Coherent and fluent procedures are in a key position considering care quality and safety.
The goal of the thesis was to improve the patient safety culture in the organization and to create a checklist corresponding to the perceptions of the contractor. The contractor of the thesis is 9Lives Oy ambulance service. The review of the literature processes the most common problems in the respiratory, circulatory and nervous systems, and introduces the most common treatments for those problems.
A checklist of the inter-hospital transfer of a critically ill patient, based on a review of up to date literature, was created as the product of this functionally implemented thesis. The subject was narrowed down into treatment of adult patients, but the checklist was aimed to suit many different medical special fields. The review of the literature was created using the most recent available research data on the subject, utilizing different data bases and textbooks. Arguments of the significant benefits of checklists considering patient safety was also included in the thesis. In the future it could be worth to investigate what impact the checklist makes on patient safety in interhospital transfers and to consider the development needs of the checklist conducted as the product of this thesis.
