Tiivistelmä
Raskaus on usein naisille jännittävä ja ainutlaatuinen kokemus. Valitettavasti kuitenkin jopa 15 prosenttia raskauksista päättyy keskenmenoon. Aiemmin koetun keskenmenon tiedetään vaikuttavan monin tavoin naisten tuleviin raskauksiin. Etenkin pelko uudesta keskenmenosta on vahvasti läsnä. Herkän, moniulotteisen ja yksilöllisen kokemuksen ymmärtäminen edellyttää lisäperehtyneisyyttä aihealueen tutkimuksiin sekä naisten omiin kertomuksiin.
Opinnäytetyön tarkoituksena oli kuvata aiemmin keskenmenon läpikäyneiden naisten pelkoja keskenmenon uusiutumisesta seuraavassa raskaudessa. Tarkoituksena oli myös tunnistaa ja kuvata keskenmenon pelkoa edistäviä ja vähentäviä tekijöitä. Tavoitteena oli tuottaa tietoa naisten hoitotyön kehittämiseksi Turun yliopistollisen keskussairaalan (TYKS) Naistenklinikalle. Lisäksi tavoitteena oli tuottaa tietoa, jota voidaan hyödyntää monipuolisesti sosiaali- ja terveysalan opetuksessa sekä neuvoloissa.
Opinnäytetyö toteutettiin laadullisena sisällönanalyysina valituista blogeista (n=10). Blogitekstit käsittelivät aiemmin keskenmenon saaneiden naisten ajatuksia keskenmenon pelosta uudessa raskaudessa.
Keskenmenon pelkoa kuvattiin epätietoisuutena tulevasta, epäluottamuksen vahvistamisena raskauden jatkumista kohtaan, raskauden salaamisena, epäsuotuisana vaikutuksena tunneelämään, tiedostettuna ja tiedostamattomana raskauden kieltämisenä sekä kielteisenä vaikutuksena henkiseen hyvinvointiin. Keskenmenon pelkoa edisti raskausoireiden muutokset tai puuttuminen sekä mahdollisuus uuteen keskenmenoon. Sen sijaan keskenmenon pelkoa vähensi suojelukseen uskominen, raskausoireiden ilmeneminen, raskauden jatkumisen vahvistaminen sekä samanlaisten kokemusten jakaminen.
Opinnäytetyön tulosten mukaan naiset kokevat vertaistuen yhtenä voimavarana uudessa raskaudessa. Keskenmenon kokeneille naisille olisikin suotuisa perustaa omia vertaistukiryhmiä. Valtakunnallisen yhtenäisen hoitolinjauksen luominen keskenmenosta mahdollistaisi tasaarvoisen hoitopolun kaikille naisille. Jatkossa tarvittaisiin lisätutkimuksia keskenmenon jälkeisen uuden odotusajan tuen tarpeen kartoituksesta neuvolan ensikäynneillä. Pregnancy is often an exciting and unique experience for women. Unfortunately, however, as many as 15 percent of pregnancies end in miscarriage. A miscarriage experienced in the past is known to affect women's pregnancies in many ways. In particular, the fear of a new miscarriage is quite intense. Understanding a sensitive, multidimensional and personal experience requires additional familiarity with researching the topic as well as with the women’s own narrations.
The purpose of this bachelor’s thesis was to describe the fears of women who had previously had a miscarrage about the recurrence of a miscarriage in the next pregnancy. The aim was also to recognize and portray the factors that promote and reduce the fear of miscarriage. The aim was to provide information for the development of women‘s nursing to the Turku University Hospital, Department of Obstetrics and Gynaecology.
This bachelor’s thesis was executed by using inductive content analysis of pre-elected blogs (n=10). Women who have miscarried before address the thoughts about the fear of miscarriage during their new pregnancy in these blogposts.
The fear of miscarriage was described as an uncertainty of the future, a reinforcement of mistrust for the continuation of pregnancy, a concealment of the pregnancy, adverse effect in the emotional state of mind, a conscious and unconscious denial of pregnancy and unfavourable effects in mental health. The fear of miscarriage was fueled by changes or the absence of gestational symptoms as well as the possibility of a new miscarriage. Instead, the fear of miscarriage was reduced by the belief in protection, the appearance of pregnancy sympotoms, confirmation of continued pregnancy, and the sharing of similar experiences.
According to the results of this bachelor‘s thesis the women consider peer support as one of the resources in a new pregnancy. It would be beneficial to set up support groups specifically for women who have gone through miscarriages. The creation of a unified nationwide care policy for miscarriage would enable an equal treatment to all women. In the future, further research would be required to identify the need of support during subsequent pregnancy in the maternity clinic visit after a miscarriage.
