Tiivistelmä
Syfte med denna studie är att kartlägga vad forskning säger om yttre stöd och lägesvård för axeln efter stroke för att minska risken för nedsatt funktionsförmåga och uppkomsten av smärta i axeln. Studien genomfördes som en systematisk litteraturstudie för att finna svar på detta. Litteratursökningen genomfördes i december 2011 och januari 2012 med hjälp av databassökningar i PEDro, EBSCO Cinahl, Pubmed och Sagepub. Utöver detta genomfördes även en manuell sökning i finska och svenska ämnestidsskrifter samt genom att granska referenserna i funna artiklar. Sammanlagt resulterade sökningen i 17 artiklar. Dessa kvalitetsgranskades och delades in i tre kvalitetsnivåer; låg, medelhög och hög.
Resultatet från studierna visar på en positiv effekt av tejpning av axeln för att fördröja uppkomsten av smärta och även för att minska funktionsnedsättningarna. Resultaten baserar sig på endast 6 artiklar, vilket gör att det inte går att dra några generella slutsatser på basen av dessa. Resultaten kring lägesvården var inte lika entydig då två artiklar visade signifikant effekt, medan fyra inte fann några sådana resultat. Artiklarna kring de övriga stöden räcker inte till för att få något enhetligt resultat då de fem artiklar som inkluderades mesta dels behandlade olika stöd och ortoser. Resultaten visar dock på en god potential vad kommer till tejpningen av axeln vilket gör detta till ett viktigt område att forska vidare inom. Ytterligare forskningen kring lägesvården och de mer moderna stöden och ortoserna, som nu visade goda resultat, behövs också före några generella slutsatser kan dras. The aim of this study is to identify what research shows about external support and
positioning for the shoulder after stroke to reduce the risk of reduced functionality and
onset of pain in the shoulder. The study was conducted through a systematic literature
review to meet the aim of my study. The literature search was conducted in december
2011 and january 2012 with the help of a database search at PEDro, EBSCO Cinahl,
Pubmed and Sagepub. In addition a manual search of Finnish and Swedish
physiotherapy magazines and the references of included articles were done. In total, the
search resulted in 17 articles. The quality of these articles were checked and divided
into three quality levels: low, medium and high.
The result shows the positive effect of taping of the shoulder to delay the onset of pain
and also to reduce functional limitations. The results were based on only 6 articles,
making it impossible to draw any general conclusions based on these. The results
around the positioning of the shoulder was not as clear when two articles showed
significant effects, while four found no such results. Articles about other supports didn’t
show enough evidence to get any consistent results when almost each of the five articles
that were included mostly dealt with different aids or orthotics. The results, however,
show good potential for taping the shoulder, which makes this an important area for
further research. Further research on the positioning and the more modern aids and
orthotics, which now showed good results, is also needed before any general guidelines
can be drawn on these.
