Tiivistelmä
Diabetes on Suomessa merkittävä kansansairaus. Yhä useampi kotihoidon asiakas sairastaa diabetesta. Ihmisen ikääntyessä voi terveydestä ja jokapäiväisestä elämästä selviäminen tarvita elämän uudelleen järjestelyjä. Hoitovastuu voi siirtyä itseltä ulkopuoliselle toimijalle, kuten kotihoidolle. Diabetesta sairastavan lääkehoidon toteutuminen kotihoidossa edellyttää sairaanhoitajalta osaamista insuliinihoidosta.
Opinnäytetyön tarkoituksena oli selvittää sairaanhoitajien ateriainsuliinihoidon osaamista kotihoidossa. Tavoitteena oli tuottaa tutkittua tietoa käytännön diabeteshoitotyöhön kotihoidossa työskenteleville osaamisen tueksi ja ohjaukseen sekä interventioiden kehittämiseen.
Tutkimusmenetelmä oli kvantitatiivinen ja aineisto kerättiin sähköisellä kyselylomakkeella. Tutkimuksen kohderyhmänä olivat perusterveydenhuollon kotihoidon sairaanhoitajat (n=15).
Tulosten mukaan sairaanhoitajien ateriainsuliinihoidon osaaminen oli vaihtelevaa. Tulosten perusteella sairaanhoitajien ateriainsuliinihoidon ja kokonaisvaltaisen diabeteshoidon osaamiseen tulee kiinnittää huomiota, jolloin voidaan taata sairaanhoitajan ammatillinen ateriainsuliinihoidon osaaminen. Ateriainsuliinihoidon osaamiseen liittyvissä perusasioissa ja erilaisten ateriainsuliinien tunnistamisessa oli puutteita. Tuloksia voidaan käyttää koulutustarpeen perusteluina koulutussuunnitelmaan ja kotihoidossa työskentelevien sairaanhoitajien ateriainsuliinihoidon toteutuksen tukemiseen.
Kyselyyn vastanneiden pääasiallinen toive oli monipistoshoitoisen diabeteshoidon koulutus. Lisäksi koulutustarve kohdistui verensokerin mittaukseen, hiilihydraattien ja ateriainsuliiniannoksen yhteensovittamiseen. Koulutuksen avulla voidaan lisätä sairaanhoitajan ammatillista päätöksentekoa ja tukea muita diabeteshoidon alueita, kuten asiakkaan ohjaus ja diabeteshoidon toteuttaminen hänen itsemääräämisoikeutensa huomioiden. Diabetes is a major public disease in Finland. More and home care clients are suffering from diabetes. As a person ages, coping with health and daily life may require reorganization of life. Responsibility for care can be transferred from oneself to an external actor, such as home care. The implementation of drug treatment for diabetes in home care requires the nurse to have expertise in insulin therapy.
The purpose of the thesis was to find out the competence of nurses in mealtime insulin therapy in home care. The aim was to produce researched information for the practical diabetes nursing for those working in home care to support and guide competence and to develop interventions.
The research method was quantitative, and the material was collected with an electronic questionnaire. The target group of the study were nurses (n = 15) in the home care of the primary health care.
The results showed that the expertise of nurses in mealtime insulin therapy varied. Based on the results, attention should be paid to the competence of nurses in insulin therapy and holistic diabetes care, which guarantees the professional competence of nurses in insulin therapy. There were shortcomings in the basics of knowledge of mealtime insulin therapy and in the identification of different meal insulins. The results can be used to justify the need for training in the training plan and to support the implementation of meal-based insulin therapy for nurses working in home care.
The main wish of the respondents was education in multi-injection diabetes care. In addition, the need for training focused on blood glucose measurement, carbohydrate and the coordination of mealtime insulin dosing. The training can be used to increase the professional decision-making nurses and to support other areas of diabetes care, such as the client's guidance and the implementation of diabetes care, taking into account his or her autonomy.
