Tiivistelmä
Syylä on papilloomaviruksen aiheuttama hyvälaatuinen ihokasvain. Se kuuluu yleisimpiin virusinfektioihin. Plantaarinen syylä (verruca plantaris) eli jalkapohjan syylä voi olla erittäin kivulias vaiva. Syylien hoito voi olla erittäin haastavaa ja pitkäkestoista. Hoitokeinoja on lukuisia ja niiden paranemistulokset vaihtelevat. Hoidon onnistumiseen on useita vaikuttavia tekijöitä, kuten henkilön ikä, immuunitila, virustyyppi ja hoitotapa.
Tämän opinnäytetyön tarkoituksena oli tehdä kuvaileva kirjallisuuskatsaus plantaarisen syylän erilaisista hoitokeinoista ja niiden vaikuttavuuksista sekä selvittää hoitoja tekevät ammattiryhmät. Tutkimuskirjallisuutta haettiin eri tietokannoista ja analysoitavaksi valikoitui 14 englannin kielistä tutkimusartikkelia. Tutkimusartikkeleista koottiin tuoretta tutkimustietoa plantaarisen syylän hoitomenetelmistä ja niiden vaikuttavuuksista.
Tieteellisten tutkimusten tuloksista selvisi, että tunnetuimmat hoitomenetelmät ovat kryo- eli jäädytyshoito sekä salisyylihappohoidot. Vaikuttavuuksiltaan ne ovat hyvin samantasoisia. Fotodynaamisten ja laser-hoitojen tehot osoittautuivat tutkimuksissa suhteellisen hyväksi. Ne ovat kalliimpia hoitoja verrattuna muihin. Laserhoidosta on raportoitu paljon haittavaikutuksia kuten arpeutumista. Syyliä hoidetaan myös sähkökirurgian avulla.
Hoitoja toteutetaan erilaisilla lääkeaineilla, kuten bleomysiinillä, joko injektoiden tai mikroneulauksella suoraan syyläkudokseen. Myös adapaleenigeeliä 0,1 %, trikloorietikkahappoliuosta sekä sidofoviiriä käytetään plantaarisen syylän hoidossa. Niitä voidaan pitää tulosten perusteella hyvinä hoitovaihtoehtoina. Adapaleenigeelillä parani vähintään 96 % plantaarisista syylistä. Liuoksilla toteutuneista hoidoista tehokkain oli yhdistelmäseos, joka koostui 1 % kantharidia, 20 % podophyllinea ja 30 % salisyylihappoa.
Kirjallisuuskatsaus kokosi toimeksiantajalle tietoa plantaarisen syylän hoitomenetelmistä ja niiden hoitotuloksista. Lisäksi selvitettiin ketkä terveydenhuollon ammattilaiset näitä hoitokeinoja käyttävät. Jatkossa voisi tehdä tutkimuskyselyn jalkaterapeuttien käyttämistä plantaaristen syylien hoitomuodoista ja mahdollisesta yhteistyöstä lääkäreiden kanssa. Kyselyn kautta selvitettäisiin syylien hoitotapoja Suomessa. A wart is a benign tumour of the skin, caused by the human papillomavirus. It is one of the most common viral infections. Plantar wart (verruca plantaris) can be a very painful affliction. The treatment of a wart can be highly chal-lenging and time-consuming. There are numerous treatments, and their heal-ing results vary. The success of the treatment is affected by many factors, such as the age and immunity status of the person, the type of the HPV infec-tion and the therapy used.
The purpose of the thesis was to provide a descriptive literature review on various treatment modalities of plantar warts and their effectiveness, and to find out the professional groups that perform the treatments. Research litera-ture was searched in various databases and fourteen research articles in Eng-lish were selected for analysis. Recent research data on plantar wart treat-ment methods and their efficacy were collected from research articles.
The studies identified cryotherapy and salicylic acid therapy as the most common treatment modalities, both of which produce very similar effective-ness. The effect of photodynamic and laser therapies proved to be relatively good in the studies. They are more expensive procedures than the other therapies. Laser therapy has been associated with numerous reports of ad-verse effects, such as scar formation. Electrosurgery is also used to treat warts.
Treatments are carried out with various medicines, such as bleomycin, which is administered with either injections or microneedling directly into the wart tis-sue. Plantar warts are treated also with adapalene, trichloroacetic acid and cidofovir. Based on the results, they can be considered to be good treatment options. Adapalene gel healed at least 96 % of warts. Regarding modalities based on the use of solutions, the most effective was a combination therapy consisting of 1 % of cantharidin, 20 % of podophylline and 30 % of salicylic acid.
The literature review gathers information for the client about the treatment methods for plantar wart and their treatments results, as well as the profes-sionals using these treatments. A topic of further study could be the chosen methods used by foot therapists for the treatment of plantar warts and their possible cooperation with medical doctors. A survey could establish the treat-ment modalities for warts from the viewpoint of foot therapy in Finland.
