Tiivistelmä
Hengitysvaikeuspotilas kuuluu ensihoidossa kymmenen yleisimmän ensihoitotehtävän joukkoon, ja kuolleisuus tässä potilasryhmässä on toiseksi suurin kaikista ensihoitotehtävistä. Hoitamatta ja huomiotta jäänyt ongelma lisää potilaiden sairaalassaoloaikaa, sekä hoitoon käytettäviä resursseja ja saattaa johtaa jopa kuolemaan. Hengityselimistön rakenne ja toiminta, sekä niissä ilmenevät haasteet esiintyvät moninaisesti ensihoitajan työssä.
Tämän opinnäytetyön tarkoituksena oli tuottaa tietotesti hengitysvaikeuspotilaan hoidosta ensihoidossa. Tietotesti koostuu videomateriaalista, case-tapauksista sekä avoimista- ja monivalintakysymyksistä. Testi on suunnattu alkuvaiheen ensihoitajaopiskelijoille ja tarkoitettu osaksi Turun ammattikorkeakoulun ensihoitajaopiskelijoiden perustason ensihoidon jaksoa. Työn tavoitteena oli muodostaa ensihoitajaopiskelijoille hyvä tietoperusta hengitysvaikeuspotilaan hoidosta ja konkretisoida potilaan hoitoon liittyviä asioita. Lisäksi tavoitteena oli auttaa opiskelijoita suorittamaan itsearviointia osaamisestaan hengitysvaikeuspotilaan hoidossa. Työ huomioi erilaisia oppijoita ja video opetusmateriaalina tukee useita eri oppimistyylejä, sillä videoista opiskelija näkee, kuulee ja lukee tietoa.
Työn kirjallisuuskatsaus päädyttiin rajaamaan ventilaatioon, happeutumiseen ja hengitystyöhön sekä niissä ilmeneviin ongelmiin ja niiden hoitoon. Esille tuotiin myös hengityselimistön anatomia ja fysiologia. Nämä aiheet on tuotu työhön ensihoidon näkökulmasta ja tietoperustan ulkopuolelle jätettiin hoitotason ensihoidon toimenpiteet, sillä työ on suunnattu perustason ensihoidon jaksolle ja kohderyhmä on alkuvaiheen ensihoidon opiskelijat.
Opinnäytetyön tuotoksena valmistui videoita sisältävä tietotesti, joka rakennettiin laajan tutkitun tiedon pohjalta. Tietotesti on laadittu Webropoliin ja antaa mahdollisuuden itseopiskeluun sekä opitun kertaamiseen. Tutkittu tieto sisällytettiin tuotokseen tiivistetysti, mutta monipuolisesti. Tietotesti on Turun ammattikorkeakoulun käytössä.
OMAT ASIASANAT: ensihoitajaopiskelija, happeutuminen, hengitystyö, hengitysvaikeus, ventilaatio Patients with dyspnoea, commonly knowns as respiratory distress, are one of the largest pa-tient groups in pre-hospital care and possess the second highest mortality rate in compared to other patient groups. An untreated and neglected respiratory problems prolongs the hospitalisa-tion and exhausts more resources and may even lead to death. The structure and function of the respiratory system with all their challenges appear abundantly in pre-hospital emergency care.
The purpose of this thesis was to produce a knowledge test on the treatment of patients with respiratory distress in pre-hospital emergency care. The knowledge test consists of videos, case-studies, open-ended questions, and multiple-choice questions. The knowledge test is di-rected towards early-stage paramedic students and to be a part of base level paramedical stud-ies in Turku University of Applied Sciences. The objective of the thesis was to form a solid foundation of knowledge in regards of treating respiratory distress and concretize of treatments. Furthermore, an auxiliary objective was to aid students in performing self-assessment on their own knowledge when treating patients with respiratory distress. The thesis acknowledges dif-ferent types of learners and video as a learning instrument supports the diverse spectrum of learning styles, due to the opportunity for a student to listen, watch and read information simul-taneously.
The literary review portion of the thesis was limited to the subjects of ventilation, oxygenation and respiration, whilst also focusing on the problems and treatments of said subjects. The anatomy and physiology of the respiratory organs was also brought into consideration. The forenamed topics were introduced from the perspective of the pre-hospital emergency care, and advanced paramedical procedures were excluded from the thesis, due to the educational nature of base-level studies and due to the thesis being directed towards early-stage students.
The thesis’ final product was a knowledge test fitted with a variety of videos and was construct-ed from expansive amount of studied information. The knowledge test was created into Webro-pol, a survey creation website, to allow the possibility of self-studying and revising. Studied in-formation was incorporated into the thesis in a compressed manner, yet diversely. The knowledge test will be used in Turku University of Applied Sciences.
