Tiivistelmä
Tämä opinnäytetyö toteutettiin kehittämistyönä. Tavoitteena oli suunnitella ja toteuttaa aistimodulaatiokoulutus mielenterveys- ja päihdekuntoutujien palveluasumisyksikön hoitohenkilökunnalle sekä tukea aistimodulaatiomenetelmän integroimisessa osaksi yksikön käytäntöjä. Toimeksiantajana toimivat Hoitokoti Peltola-Sävelkuja Oy:n toimintaterapeutti ja yksikön johtaja.
Englanninkielisellä käsitteellä sensory modulation viitataan kontekstista riippuen joko ihmisen neurofysiologiseen prosessiin (aistisäätely) tai terapeuttiseen menetelmään (aistimodulaatio). Aistimodulaatiomenetelmällä tarkoitetaan näyttöön perustuvaa interventiota, jonka avulla voidaan vaikuttaa ihmisen olotilaan: aistiärsykkeen aiheuttama muutos fysiologiassa voi johtaa muutokseen ihmisen tunne- tai vireystilassa. Aistimodulaatiomenetelmä on tunnistettu tehokkaaksi tavaksi uudistaa mielenterveyspalveluita ja parantaa sen laatua, ja sen implementoimisella osaksi ydinkäytäntöjä on todettu pitkälle kantavia hyötyjä niin asiakkaille, heidän perheilleen ja huoltajilleen, kuin palvelujen tuottajillekin.
Koulutuksen sisällön suunnittelussa hyödynnettiin teoriaa aisteista, aistimodulaation neurofysiologisesta prosessista sekä aistimodulaatiosta terapeuttisena menetelmänä. Rakenteen suunnittelun taustalla vaikuttivat oppimisen ja motivaation teoriat, kuten autenttinen oppiminen. Implementoitumisen elementti huomioitiin peilaamalla työskentelyä implementoinnin CFIR-malliin ja sitä tukevaan koulutuksen siirtovaikutuksen malliin. Kehittämistyöprosessin tukena hyödynnettiin konstruktivistista mallia, minkä vuoksi työskentelyssä korostuivat reflektiivinen työote ja avoin vuorovaikutus toimeksiantajan kanssa.
Kehittämistyön tuloksena syntyi kolmen tunnin koulutuskokonaisuus, johon sisältyi koulutusdiasarja, toiminnallisia tehtäviä ja koulutuksen jälkeen suoritettavia välitehtäviä. Palautekyselyn vastausten perusteella työntekijöiden ennakkokäsitykset aistimodulaatiosta muuttuivat positiivisesti. Onnistunut aistimodulaatiomenetelmän implementointi voi tukea asumisyksikön asukkaiden ja työntekijöiden hyvinvointia sekä antaa yritykselle lisäarvoa, minkä vuoksi mm. koulutusmateriaali ja kehittämisideat jäävät yrityksen käyttöön tukemaan prosessin jatkumista. This bachelor’s thesis is a development project for Hoitokoti Peltola-Sävelkuja Oy which provides supportive housing and care for patients with mental health and substance abuse issues. The objective was to plan and produce a training about sensory modulation for the nursing staff, and support the company in implementating the use of sensory modulation into their daily practice. The occupational therapist and the the head of the unit were the employers of this project.
Depending on the context the term ”sensory modulation” may refer to either the neurophysiologial process or to an intervention. The sensory modulation intervention is an evidence-based intervention that works by changing how a person feels through sensory input. This input changes the physiology of the body and in doing so can create a shift in a person’s emotional state or level of alertness. Sensory modulation has been formally acknowledged to be an effective way of reforming and improving the quality of mental health services, and the implementation into the core practice has been said to have far reaching benefits for the clients, their families, carers and service providers.
To support the planning of the training content, theory about senses, sensory modulation intervention and its neurophysilogical background was used. The theories of learning and motivation, for example the elements of authentic learning, affected what kind of structure and tasks were used. The implementation element of the project was consider by reflecting the work on CFIR implementation model, and by using the model of transfer which has to do with the generalization and maintance of material learnt in training to the work environment. The constructive process model was used in the background of the development project which is why reflective working methods and open communication with the employer were emphatized.
The result of the project was a three-hour training which included a powerpoint, activating tasks and posttraining tasks. Results of the feedback survey show a positive change in nursing personnel’s attitudes towards sensory modulation. Successful implementation of sensory modulation can support the wellbeing of residents and the nursing staff, and give additional value to the company’ services. Thus training material and other development suggestions were left to the employer and the nursing staff to support the continuity of the implementation process.
