Tiivistelmä
Syftet med studien har varit att identifiera specifika behov hos patienter med psykisk sjukdomshistoria i behov av somatisk akutvård. Vid akuta vårdsituationer är tiden begränsad, omgivningen inverkar på situationen samt vårdaren bör beakta patientens mentala och nuvarande status.
Centrala frågeställningar i studien är: Vad bör sjukskötare på en jourpoliklinik veta om psykiatriska patienter för att kunna erbjuda dem så god vård som möjligt? Bidrar insikt om specifika behov hos patienter med psykisk sjukdomsbakgrund till att sjukskötares attityder emot den psykiatriska patientgruppen neutraliseras?
Den teoretiska bakgrunden i studien behandlar bemötande av patient, mötet mellan sjukskötare och patient, kommunikationens betydelse i vårdsituationen. För att skapa bättre inblick i situationen beskrivs även arbetet på en jourpoliklinik och vad Triage-bedömning innebär. Som teoretiska referensramar har använts den interpersonella processen enligt Peplau (1991) samt tankar kring vårdrelationen (Kasén 2002).
Studien är en kvalitativ teoristudie baserad på flertalet vetenskapliga relevanta artiklar. Datainsamlingen har skett genom dokumentstudier. Artiklarna är sammanfattade i en resuméartikel (Kirkevold 1997) för att skapa överskådlighet. Som dataanalysmetod har använts kvalitativ innehållsanalys (Kyngäs & Vanhanen 1999).
Centrala resultat i studien har varit att professionellt förhållningssätt, god kommunikation samt kunskap om psykisk sjukdom är faktorer som underlättar arbetet för sjukskötare vid bemötande av patienter med psykisk sjukdom i bakgrunden. Vidare har framkommit att attityder påverkar beteendet, sjukskötarens syn på hälsa och välmående inverkar på erbjuden vård. The aim of this thesis was to determine, through a theoretical qualitative study of literature, specific needs among patients with a psychiatric illness background who are in need of somatic emergency care. Data was collected through a document study andin order to create lucidity the data was summarized in a review article. Research articles were analyzed by means of content analysis.
The research questions where: What should nurses within the emergency unit know about the psychiatric patient to be able to provide the best possible care? Does specific knowledge about the needs of patients with a psychiatric diagnosis contribute to less negative attitudes towards these patients?
The nurse-patient relationship and the significance of good communication in care situations make up the theoretical background of this study. Also the work at the emergency department is described as well as triage assessment. The theoretical starting points are Peplau’s (1999) interpersonal relation process theory and Kasén’s (2002) thoughts on caring within the relationship.
The results of this study are following: Factors that facilitate the situation when caring for patients with psychiatric issues are a professional approach, good communication skills and knowledge about psychiatric illness. Furthermore, the results of the study show that attitude affects behavior and the nurse’s opinion on health and wellbeing affect the care that is offered.
