TIIVISTELMÄ
Tämän opinnäytetyön aiheena oli TRAM-potilaan postoperatiivinen eli leikkauksen jälkeinen hoito. Opinnäytetyö toteutettiin toiminnallisena opinnäytetyönä. Työn tarkoituksena oli tuottaa näyttöön perustuva kotihoito-ohje TRAM-potilaalle. Työn tavoitteena oli edistää potilaan toipumista TRAM-leikkauksen jälkeen, antamalla potilaalle tukea kotona selviytymiseen suullisen ohjauksen lisäksi. Työn tilaajana toimi Kanta-Hämeen keskussairaalan kirurginen osasto. Kotihoito-ohje toteutettiin tilaajan toiveet huomioiden.
Opinnäytetyön tietoperustaan kerättiin tietoa rintasyövän hoidosta, TRAM-leikkauksesta, TRAM-potilaan postoperatiivisesta hoidosta ja TRAM-potilaan ohjaamisesta sekä tukemisesta. Tietoa kerättiin verkosta tietokantojen avulla ja kirjoista manuaalisesti. Lisäksi tilaajan toiveesta tietoa etsittiin TRAM-potilaan postoperatiivisesta fysioterapiasta. Fysioterapian tiimoilta oltiin yhteydessä myös Kanta-Hämeen keskussairaalan plastiikkakirurgian fysioterapeuttiin.
Opinnäytetyön toiminnallisena tuotoksena syntyi kotihoito-ohje TRAM-potilaalle. Kotihoito-ohjeesta tuli tiivis ja selkeä kokonaisuus. Tilaaja antoi tuotoksesta positiivista palautetta. Ohjeen on tarkoitus tulla Kanta-Hämeen keskussairaalan TRAM-potilaiden käyttöön. Opinnäytetyöprosessi-luvussa kuvattiin opinnäytetyön toteutusta ja kotihoito-ohjeen luomista. Opinnäytetyön sisältöä, eettisyyttä ja luotettavuutta tarkasteltiin pohdinnassa. Opinnäytetyötä ohjasivat vahvasti HAMKin hyvän tieteellisen käytännön ohjeet. ABSTRACT
The subject of this Bachelor’s thesis was postoperative treatment of TRAM-patient. The Bachelor’s Thesis was done as practice based thesis. The purpose of this work was to produce evidence based home treatment instructions for the TRAM-patient. The aim of the work was to help the progress of healing after the patient’s TRAM-operation, by giving the patient spoken instructions and also support for home survival. The commissioner of the work was surgical ward of the Kanta-Häme central hospital. The home treatment instructions were done by taking into consideration the commissioner’s wishes.
For Bachelor’s thesis database the information of breast cancer treatment, TRAM-operation, TRAM-patient’s postoperative treatment and the guidance and support of the patient were collected. The background information was collected from online databases and books. As for the commissioner’s wishes, also the background information of the patient’s postoperative physiotherapy was collected. Plastic surgical ward of the Kanta-Häme central hospital was also consulted for the physiotherapy.
The practise based outcome of the Bachelor’s thesis was the home treatment instruction for the TRAM-patient. Outcome of the home treatment instruction was compact and clear entity. The commissioner gave positive feedback for the outcome. Instruction is meant to come for the use of Kanta-Häme central hospital TRAM-patients. In process of the Bachelor’s thesis, the making of Bachelor’s thesis and home treatment instruction was described. Content, ethics and reliability of the Bachelor’s thesis were examined. The thesis was guided by good scientific practice instructions of Häme University of Applied Sciences.
