Tiivistelmä
Tämän opinnäytetyön tavoitteena oli selvittää, kuinka tukea potilasta tupakoinnin lopettamisessa ennen leikkaukseen tuloa. Tarkoituksena oli parantaa potilasohjausta sekä tuottaa hoitohenkilökunnalle potilasohje ohjaustyön tueksi. Opinnäytetyön näkökulmina toimivat anestesiahoitotyö ja preoperatiivinen ohjaus erikoissairaanhoidossa. Tarve työlle kumpusi suoraan työelämästä, sillä tupakoinnin aiheuttamat leikkausriskit näkyvät myös anestesiahoitotyössä. Työn tilaajana toimi Kanta-Hämeen keskussairaalan kirurgian poliklinikka.
Opinnäytetyö toteutettiin toiminnallisena opinnäytetyönä, jonka teoreettinen viitekehys koostettiin ajantasaiseen tutkittuun tietoon perustuen. Tietoa kerättiin itse tupakasta ja sen vaikutuksista potilaaseen sekä anestesiaan. Jotta näitä voitiin ymmärtää, opinnäytetyössä avattiin hengityselimistön anatomiaa ja fysiologiaa sekä sivuttiin verenkiertoelimistöä ja sen toimintaa. Tämän jälkeen lähdettiin keräämään tietoa tupakoinnista sekä riippuvuudesta, sen hoidosta ja potilasohjauksesta. Tietoa kerättiin erilaisista tietokannoista, hakupalveluista ja hoitosuosituksista, perinteistä kirjastohakua unohtamatta. Tiedonhakua rajatiin työn identifioinnilla, opinnäytetyökysymyksillä sekä tiedonhaun vaiheessa lähteiden seulonnalla.
Teoreettisen viitekehyksen pohjalta voidaan todeta tupakoinnin moninaiset epäsuotuisat vaikutukset sairastavuuteen ja kuolleisuuteen, mutta samalla nähdä myös laajat vaikutusmahdollisuudet hoitopolun eri vaiheissa. Hyvä potilasohjaus on motivoivaa ja se perustuu riittävään tietoon aiheesta. Opinnäytetyön tuloksena syntyi kaksi A4-kokoista potilasohjetta, joista toinen suunnattiin pelkästään hoitohenkilökunnalle ja toinen sekä hoitohenkilökunnalle että potilaille. Hoitohenkilökunnalle suunnatun ohjeen tarkoituksena on tukea ohjaustilanteen etenemistä muistilistan tavoin, kun taas hoitohenkilökunnalle ja potilaille tarkoitetun ohjeen ajatuksena on tuoda selkeällä kielellä esille tupakointiin ja anestesiaan liittyviä riskejä. Työssä huomioitiin tilaajan toiveita ja toimittiin yhteistyössä läpi opinnäytetyöprosessin. Jatkotutkimuskysymykset voisivat koskea toiminnallisen osuuden käytettävyyttä ja kehitysideoita. The aim of this Bachelor’s thesis was to study how to support a patient with smoking cessation before entering surgery. The purpose was to study how to improve patient guidance and produce instructions for the medical staff to support their work. In this thesis, the used perspectives were anaesthesia nursing and preoperative guidance in specialized medical care as the idea for this study was found during a practical training in operating theatres in Kanta-Häme central hospital. After preceding studies, the surgical outpatient clinic was found as a commissioner for this thesis.
The study was carried out as a practice-based thesis, which theoretical framework was based on nursing and medical knowledge and the main points were evidence-based nursing and multidisciplinary care. The information was collected about tobacco and its effects on the patient, as well as to anaesthesia. To understand these, the thesis discussed anatomy and physiology of the respiratory system. At the final part of the study, the information was gathered about smoking, addiction, treatment options and how to support smoking cessation. Various databases, search services and national instructions were a part of the information retrieval process, not forgetting the traditional library search. Defining the thesis was used as an aid in research. That included thesis questions which were: What risks does smoking cause during anaesthesia? How do the risks of smoking affect the patient's condition during the procedure? How to support and guide the patient in smoking cessation in the preoperative stage in special health care?
Conclusions based on this study were that smoking causes morbidity and mortality in many ways and those affects are increasing the risks in anaesthesia. These risks are harming the recovering process from the surgery mainly through weakened oxygenation, challenging pain management and increased infection risk. It is possible to lower the risks 30-40 percent by smoking cessation. Effective support was patient centered and based on sufficient information. Based on this knowledge, two A4 sized patient guidelines were made, one for nurses and one for the patients. Further research questions could apply to the usability of the guidelines and development proposals.
