Syftet med detta examensarbete är att skapa en bild av huruvida förstavårdare känner sig bemästra vården vid prehospitala förlossningar. Då mindre förlossningsenheter har stängts, vården koncentreras till större enheter och därav avstånden till sjukhusen ökat har även de prehospitala förlossningarna ökat. Dessa sköts oftast av förstavårdare på fält och frågeställningen med detta arbete är därav ”hur upplever förstavårdarna sin kompetens och trygghet i sin yrkesroll vid prehospitala förlossningar?” ”Vilka saker kan pekas ut som enligt förstavårdarna kunde stärkas och vilka saker känner de att de behärskar?” Arbetet tar upp stängningarna av förlossningsenheterna och dess inverkan på vården av förlossningar och hur detta ändrar förstavårdarnas arbetsbild. Som teoretisk referensram används begreppet kompetens. Metoden som används är en scoping studie, som är en översiktsstudie där man genom att studera i redan befintlig vetenskaplig litteratur skapar en bild av vad som redan forskats i och vad som behöver forskas mera i. Till studien valdes artiklar som var skrivna på svenska, finska eller engelska, samt skrivna tidigast år 2012. Till studien valdes slutligen fem artiklar som innehöll för studien relevant information. Före valet lästes artiklarna i sin helhet. De fem valda artiklarna analyserades individuellt och resultatet av studien av dessa kompletterades med ett sammandrag och diskussion. Resultatet av studien visar att det finns tämligen lite vetenskaplig forskning inom ämnet och att det finns ett tydligt behov av mera forskning. Det finns även tydliga tecken på att förstvårdarna känner att de inte alltid bemästrar vården av en prehospital förlossning och att det finns ett behov av mera utbildning och handledning i prehospitala förlossningar.
The purpose of this bachelor thesis is to create a view of the feeling of competence and confidence among paramedics dealing with prehospital births. When smaller maternity units have been closed down and the births are concentrated to larger hospitals, it also automatically leads to longer distances and time of transport. A consequence of that is an increased amount of prehospital births, happening either at home or on the way to the hospital. The prehospital births are mostly managed by paramedics, and therefore the research question of this study is “does paramedics feel confident and safe in their competence when dealing with prehospital births?” and “what aspects do according to paramedics need improvement and what do they think they already have the needed skills for?” The study addresses the background considering the closing of maternity units and the impact of that on births and paramedics work field. The theoretical frame used in this study is competence. The method used is a scoping review, a kind of literature study where the aim is on creating a wide perspective on what already has been studied and possible subjects that should be researched more in the future. The articles selected for this study were written in Swedish, Finnish or English and published earliest in 2012. Finally five articles, containing relevant information for this study, were used. The articles used were first analyzed individually and then a conclusion was made at the end. The result of this study was that there are very few studies made about the matter, therefore not much material is to be found. There is also a need of more research in the field of prehospital births and paramedics dealing with that. The message from the field is quite clear; There is a need for more education, practical training and mentoring in paramedics dealing with prehospital births.
