Laitoshoidon muuttaminen tehostetun palveluasumisen yksiköksi : muutosta tukeva selvitys

Abstract

Ikääntyneiden osuus väestöstä kasvaa, mikä lisää pitkäaikaishoidon tarvetta. Pitkäaikaishoidon järjestämisen tulee olla asiakaslähtöistä ja omaisia osallistavaa. Ikääntyneiden pitkäaikaishoitoa toteutetaan laitoshoidossa ja asumispalveluissa. Hallitus on esittänyt, että laitoshoidon järjestäminen päättyisi vuoden 2027 loppuun mennessä.

Tämän kehittämisprojektin tarkoituksena oli laatia selvitystyö ikäihmisten laitoshoidon muuttamiseksi tehostetun palveluasumisen yksiköksi Laitilan kaupungissa. Tavoitteena oli vastata työelämän tarpeeseen ja tuottaa muutosta tukeva selvitys.

Kehittämisprojektin toteutusvaiheessa tehtiin benchmarking-vertailukehittäminen kahden tehostetun palveluasumisen yksikön muutosprosesseista. Vertailukehittämisen tuloksena laadittiin tarkistuslista muutosprosessia varten. Tarkistuslistan teemoiksi muodostuivat yksikkö, johtaminen, henkilöstö, asukkaat ja omaiset/läheiset. Listaa täydennettiin lähdekirjallisuuden avulla. Kirjallisuuskatsauksesta tarkastuslistalle nostettiin kodinomaisuuden ja vanhuuden erityispiirteiden huomioiminen, muutosviestintä, palvelu-, hoito- ja kuntoutussuunnitelman tärkeys, hoitotyön asukaskeskeisyys, hoitohenkilöstön ja omaisten osallistaminen sekä arvot itsemääräämisoikeus ja yksilöllisyys. Muutosprosessiin liittyvä kirjallisuuskatsaus on osa tämän kehittämisprojektin tuotosta.

Vertaiskehittämisen ja lähdekirjallisuuden avulla tuotetulla muutosprosessin tarkistuslistalla on käytännön hyöty vanhustyössä. Tuotos on hyödynnettävissä missä tahansa organisaatiossa suunniteltaessa laitoshoidon muuttamista tehostetun palveluasumisen yksiköksi.

As the number of elderly people in the population is growing, also the need for long-term care is increasing. Long-term care must be customer-oriented and involve relatives in the care. Long-term care for the elderly is provided as institutional care and service housing. The Finnish Government has proposed that providing institutional care should end by the end of the year 2027.

This thesis is implemented as a development project. The purpose of the project is to prepare a study of changing a nursing home into an intensive service housing unit with 24-hour care in the town of Laitila. The aim of the project is to respond to working life needs and to produce a study to support the change.

The development method used in this project is benchmarking. Benchmarking is conducted between the reorganisation processes of two service housing units which offer 24-hour assistance. A checklist for the reorganisation process has been formulated on the basis of the benchmarking process. The themes of the checklist concern the care unit, its management, the personnel, the residents and their relatives. The list has been supplemented on the basis of the research literature. The themes brought in from the literature are the special characteristics of the elderly, the coziness of the unit, change management communication, the importance of advance care planning, nursing focused on the resident, the inclusion of nursing staff and relatives in the care and self-determination and individuality as central values in care. The literature review related to the reorganisation process is part of the results of this development project.

The checklist of the reorganisation process has practical benefits in the care of the elderly. The results can be utilised in any organisation which plans to reorganize institutional care into intensive service housing.
