Mielenterveys- ja päihdehoitotyön integraatio: integroidun hoidon toteutuminen hoitohenkilökunnan kokemana

Abstract

Mielenterveys- ja päihdepalveluiden kehittyessä palveluita ja hoitoa on integroitu eli yhdistetty, ja tästä on syntynyt integroitu hoitomalli. Integroidun hoitomallin myötä kaksoisdiagnoosipotilas saa hoidon molempiin häiriöihinsä samasta paikasta ja samaan aikaan. Integroitu hoitomalli on tuotu Suomeen Mieli-suunnitelman myötä vuonna 2009. Opinnäytetyössä tarkastellaan integroidun hoidon toteutumista hoitohenkilökunnan näkökulmasta. Tarkoituksena oli kartoittaa mielenterveys- ja päihdepalveluiden hoitohenkilökunnan kokemuksia integroidun mielenterveys- ja päihdehoitotyön toteutumisesta. Tavoitteena oli saada tietoa integroidun hoidon toteutumisesta hoitohenkilökunnan näkökulmasta. Tutkimuskysymyksinä olivat, toteutuuko integroitu mielenterveys- ja päihdehoitotyö hoitohenkilökunnan näkökulmasta sekä kokeeko hoitohenkilökunta mielenterveys- ja päihdehoitotyön integraation positiivisena kehitysaskeleena ja jos ei, miksi.

Opinnäytetyö toteutettiin määrällisenä tutkimuksena Webropol-työkalulla luotua kyselyä käyttämällä. Kysely suunnattiin Itä-Savon sairaanhoitopiiri Ky:n aikuisille suunnattujen mielenterveys- ja päihdepalveluiden hoitohenkilökunnalle. Kyselyyn tuli vastauksia 19 (N=19) ja vastausprosentti oli 10,45 %. Tulokset analysoitiin Webropolin-raportointityökalun avulla ja avoimet kysymykset analysoitiin aineistolähtöisen sisällönanalyysin avulla.

Tulosten mukaan suurin osa hoitohenkilökunnasta kokee integraation positiivisena kehitysaskeleena. Kaikki vastaajat kokivat tarvetta integraation jatkokehittämiselle. Kehityskohteiksi muodostuivat hoitomallit ja hoitopolut, moniammatillinen yhteistyö, avohoito sekä hoitohenkilökunnan lisäkoulutus. Hoitohenkilökunnan kokemuksista nousee esiin tämänhetkinen päihdekuntoutuksen heikko tilanne, jonka ei koeta olevan kuntoutujaa kannattelevaa. Suurin osa vastaajista koki olevansa osittain samaa mieltä integroidun hoidon toteutumisesta.

Jatkotutkimusehdotuksena on kartoittaa hoitohenkilökunnan kokemuksia myö-hemmin kehityksen seuraamiseksi. Toisena jatkotutkimusehdotuksena on kartoittaa mielenterveys- ja päihdeasiakkaiden kokemuksia integroidusta hoidosta. Jatkokehittämisehdotuksena on kehittämisentarpeen seurantalomake.

As mental health and substance abuse services have evolved, the services and treatment have been integrated, which has resulted in an integrated treatment model. With the integrated treatment model, a dual-diagnosis patient receives treatment for both of their disorders, from the same place and at the same time. The integrated care model was brought to Finland in 2009 with the national plan for mental health and substance abuse work, Mieli. The study examines the implementation of integrated care from the point of view of nursing staff. The purpose was to survey the experiences of nursing staff in Mental Health and Substance Abuse Services regarding the implementation of integrated mental health and substance abuse nursing. The goal was to obtain information about the implementation of integrated care from the point of view of the nursing staff. The research questions inquired whether integrated mental health and substance abuse nursing is realized from the perspective of the nursing staff, and whether the nursing staff experience integration in mental health and substance abuse nursing as a positive development step, and if not, why. The study was carried out as a quantitative study using a questionnaire created using the Webropol tool. The survey was directed to the medical staff of the Mental Health and Substance Abuse Services for adults in the Eastern-Savo Hospital District. The survey received 19 responses (N=19) and the response rate was 10.45 %. The results were analysed using the Webropol reporting tool and open questions were analysed using data-based content analysis. According to the results, most of the medical staff perceive integration as a positive development step. All respondents felt the need for further integration development. Treatment models and paths, multidisciplinary cooperation, outpatient care and additional training of the nursing staff were mentioned as targets for development. The experience of the nursing staff highlights the current poor situation in substance abuse rehabilitation, which is not perceived to be supportive of the rehabilitation client. Most of the respondents felt that they were partially in agreement with the implementation of integrated care. A further research proposal is to survey the experiences of nursing staff later to track development. Another further research proposal is to survey mental health and substance abuse clients' experiences in integrated care. A proposal for further development is a follow-up form for the need for development.
