Tiivistelmä
Opinnäytetyön tavoitteena oli kerätä tietoa siitä, kuinka paljon ja kuinka usein hoitajat Kanta-Hämeen keskussairaalassa kohtaavat väkivaltaa tai sen uhkaa työssään. Lisäksi haluttiin selvittää, kuinka tehokkaina hoitajat pitivät voimassa olevia väkivaltaa ehkäiseviä turvallisuus- ja varotoimia, sekä mitä osapuolia turvallisuuden edistämiseksi heidän mielestään tulisi parantaa. Opinnäytetyön tilaajana toimi Kanta-Hämeen keskussairaala.
Opinnäytetyön teoriaosuus koostuu väkivallan määrittelystä, sen vaikutuksista sekä käsitellään väkivallan ehkäisyä sekä väkivaltatilanteiden purkamista ja ehkäisytoimien kehittämistä. Opinnäytetyö on tyypiltään tutkimuksellinen. Opinnäytetyö toteutettiin kvantitatiivisena tutkimuksena. Tutkimuksessa selvitettiin väkivallan tai sen uhan esiintymistä vuodeosastoilla ja päivystyksessä, kuinka hyvin hoitajat tuntevat oman osastonsa käytössä olevat turvallisuus- ja varotoimet sekä miten näitä voitaisiin hoitajien mielestä kehittää.
Hoitajille suunnattuun kyselyyn saatiin lopulta 96 vastausta. Kysely toteutettiin verkkolomakkeella ja se on tyypiltään puolistrukturoitu. Avoimet vastaukset pelkistettiin ja ryhmiteltiin, jonka jälkeen ne esitettiin teemoittain. Numeeriset vastaukset käsiteltiin Webropol-työkalulla ja Exceliin muodostetulla havainnointimatriisilla.
Tutkimuksessa selvisi, että suurin osa vastaajista kokee potilaan tekemää henkistä ja fyysistä väkivaltaa kuukausittain. Omaisten tekemää henkistä väkivaltaa koettiin muutamia kertoja vuodessa ja fyysistä väkivaltaa kohdattiin harvoin ja nämä tapaukset keskittyivät päivystykseen. Vastanneista puolet ovat käyneet työnantajan vaatimat turvallisuuskoulutukset, osa ei ole käynyt minkäänlaisessa koulutuksessa. Vastanneet kokivat työympäristönsä kohtuullisen turvalliseksi ja väkivalta harvemmin johtaa työkyvyn laskuun. Suurin osa vastaajista tekee väkivaltatilanteesta HaiPron sekä ilmoittaa siitä esimiehelleen. Valitettavasti kolme neljästä ei ole ilmoituksen jälkeen nähnyt parannuksia työympäristön turvallisuudessa. Kolme kohtaa turvallisuudesta, jotka vaativat vastaajien mielestä parantamista, ovat perehdytys ja työnohjaus, väkivallan ehkäisyyn kohdennettu koulutus sekä väkivaltariskien tunnistaminen. The thesis was aimed to collect information on how much and how often the nurses at Kanta-Häme Central Hospital encounter violence. We wanted to find out how effective the safety and precautions to prevent violence in place were, as well as which parts should be improved to promote safety. Kanta-Häme Central Hospital was the client of the thesis.
The theory part of the thesis consists of the definition of violence, its effects, and prevention, as well as the resolution of violent situations and the development of prevention measures. A quantitative study was conducted in the thesis. The study investigated the occurrence of violence or the threat of it in inpatient wards and in the emergency room, how well the nurses know the safety and precautions in use in their department, and how, in the nurses' opinion, these could be improved.
96 responses to the survey were received. The survey was implemented using an online form and is semi-structured in type. The open-ended answers were reduced and grouped, after which they were presented by theme. The numerical answers were processed with the Webropol tool, and an observation matrix was created in Excel.
The survey revealed that the majority of respondents experienced mental and physical violence perpetrated by a patient monthly. Emotional violence by family members was experienced a few times a year. Physical violence by family members was rarely encountered and these cases were concentrated in the emergency room. Half of the respondents have attended the safety training required by the employer; some have not attended any kind of training at all. The respondents perceived their work environment to be reasonably safe and violence rarely lead to a decreased ability to work. Most of the respondents submitted a HaiPro and reported the violent situation to their supervisor. Sadly, three out of four did not see improvements in the safety of the work environment. Based on the survey, the three points on security that need the most improvement in the respondents' opinion are orientation and job counseling, targeted training for violence prevention, and identification of violence risks.
