Tiivistelmä
This thesis was done as a development project for improving patient safety, by forming a checklist to be used as a mnemonic tool during the patient admission process in a palliative health care ward. The aim of the thesis was to develop the patient admission process in basic healthcare for safer, more efficient and coordinated patient - nurse interaction. This action-based development project was carried out by mostly using a primary collection method, which was through observation, working at the unit, and conversation with colleagues. This information was supported by using a secondary data collection method, gathering background information from books, journals, articles, and legal text.
The conclusions reached from this thesis were that the use of mnemonic tools improve nursing safety, and that the created checklist is a versatile tool that can be used as reference in other similar basic healthcare settings.
