Esitietolomake lapsen toimintaterapia-arvioon Etelä-Pohjanmaan hyvinvointialueella: tutkimuksellinen kehittämistyö

Abstract

Opinnäytetyö toteutettiin Etelä-Pohjanmaan hyvinvointialueen toimintaterapiayksikköön kehittämishankkeena hyvinvointialueiden aloittaessa toimintansa 1.1.2023. Opinnäytetyön tavoitteena oli laatia esitietolomake, jonka avulla perusterveydenhuollon toimintaterapeutin arvioon tulevan lapsen lähiaikuisilta voitaisiin kerätä tietoa. Tarve yhtenäiselle esitietolomakkeelle oli noussut alueen toimintaterapeuttien verkostotapaamisissa.

Opinnäytetyön teoreettisina lähtökohtina olivat toimintaterapiaintervention prosessimalli (OTIPM) ja toiminnan transaktionaalinen malli (TMO). Muina teoreettisina rajauksina käytettiin toimintaterapia osana perusterveydenhuoltoa, lasta toimintaterapian asiakkaana ja toimintaterapian alkuvaiheen tiedonkeruuta arvioinnin suunnittelun tukena. Tutkimuksen tiedot kerättiin Etelä-Pohjanmaan hyvinvointialueen perusterveydenhuollossa lasten toimintaterapia-arviointeja tekeviltä toimintaterapeuteilta anonyyminä Webropol-kyselytutkimuksena. Kerättyjen tietojen pohjalta tehdyn analyysin avulla koostettiin Etelä-Pohjanmaan hyvin-vointialueelle esitietolomake lähetettäväksi arvioon tulevien lasten lähiaikuisille.

Tämän tutkimuksen perusteella Etelä-Pohjanmaan hyvinvointialueen perusterveydenhuollon lasten toimintaterapeutit haluavat saada esitietoja lapsen kehityksen kulusta, lapsen valmius- ja taitotason tekijöistä sekä toiminnassa erilaisissa sosiaalisissa ja fyysisissä ympäristöissä näkyvistä haasteista. TMO:n tilanne- sidon-naisissa elementeissä halutut tiedot sijoittuvat asiakkaan elementteihin, sosiaalisten ja fyysisten ympäristöjen elementteihin sekä ajallisiin elementteihin. Esitietolomake koettiin hyödyllisenä työvälineenä näiden tietojen keräämiseen.

Esitietolomakkeen avulla voidaan kehittää toimintaterapian arviointivaihetta tasa-arvoisemmaksi ja yhtäläiseksi koko hyvinvointialueella. Käyttökokemuksen myötä nähdään myös esitietolomakkeen vaikutus kustannustehokuuteen. Kehittämisideoina esitettiin lomakkeen viemistä sähköiseen muotoon sekä lomakkeen käyttöönoton pilotointia.

The thesis was carried out in the occupational therapy unit of the South Ostrobothnia wellbeing services coun-ty as a development project as the new wellbeing services counties started their operations on 1 January 2023. The aim of the thesis was to prepare a preliminary information form to collect information from the next of kin of a child who is being assessed by a primary care occupational therapist. The need for a standard-ized preliminary information form had arisen at network meetings of occupational therapists in the South Ostrobothnia area.

The theoretical starting points of the thesis were the Occupational Therapy Intervention Process Model (OTIPM) and the Transactional Model of Occupation (TMO). Occupational therapy as part of primary health care, the child as an occupational therapy client, and data collection in the initial information gathering phase as a support for planning the occupational therapy evaluation were also included in the theoretical basis. The data was collected from the occupational therapists performing occupational therapy evaluations of children in the primary health care of the South Ostrobothnia wellbeing services county with an anonymous Webropol survey. The collected data was used to compile a preliminary information form for the South Ostrobothnia wellbeing services county to be sent to the next of kin of the children who will be evaluated.

The study shows that the occupational therapists who responded to the survey want to get preliminary infor-mation about the course of the child’s development, the factors of the child's ability and skills, as well as the challenges visible in activities in different social and physical environments. In the situational elements of TMO, the desired information concerns the client elements, elements of social and physical environment and temporal elements. The preliminary information form was perceived as a useful work tool.

The preliminary information form can guide occupational therapists to plan the assessment phase and devel-op the evaluation phase to be more equal in the entire wellbeing services county. With user experience, the impact of the preliminary information form on cost efficiency will be seen. As a further study, an electronic version of the form could be created, and implementation piloted.
