Videokonsultaation mahdollisuudet osana ensihoidon hoito-ohjeprosessia

Abstract

Tutkimuksien mukaan ensihoidon kiireettömät tehtävät ovat lisääntyneet ja potilaat voitaisiin usein ohjata jäämään kotiin tai muuhun tarkoituksenmukaisempaan terveydenhuollon palveluun kuin päivystykseen. Hoito-ohjeprosessi sisältää muun muassa hoidon tarpeen, hoidon suunnittelun ja sen tavoitteiden arvioinnin. Hoito-ohjeprosessissa ensihoitaja tekee usein lisäksi konsultaatiopyynnön lääkärille puhelimitse. Tämän tutkimuksen tarkoituksena oli selvittää videokonsultaation mahdollisuuksia ja haasteita osana ensihoidon hoito-ohjeprosessia (konsultaatiota). Tutkimuksen tavoitteena on tutkia, mitä vaikutuksia videokonsultaatioilla olisi konsultaatioiden onnistumiseen ja potilaan hoidon laatuun, sekä tuottaa tietoa, jota voitaisiin jatkossa hyödyntää hoito-ohjeprosessin (konsultaatioiden) kehittämisessä. Tämän tutkimuksen aineistoa kerättiin laadullisella kyselytutkimuksella. Kysely toteutettiin sosiaalisessa mediassa avoimena kyselynä Webropol-ohjelmalla ensihoidossa työskenteleville ammattilaisille. Kyselyyn vastasi viisikymmentä henkilöä. Kyselystä saadun aineiston analysoinnissa käytettiin induktiivista eli aineistolähtöistä sisällönanalyysiä. Tutkimuksen tuloksien mukaan ensihoitajien näkemykset videokonsultaatioiden mahdollisuuksista osana ensihoidon hoito-ohjeprosessia liittyivät hoidon tarpeen arviointiin, videoyhteyksiin viestinnän tukena, fyysisten vammojen arviointiin ja hoito-ohjeisiin videoyhteyden avulla. Näiden lisäksi videoyhteydestä voisi olla tukea ensihoitajan tehdessä kuljetuspäätöksiä. Lisäksi ensihoitajat toivat esiin näkemyksiään videokonsultaatioiden haasteista, jotka liittyivät sen koulutukseen ja implementointiin, videoyhteyksien teknisiin ongelmiin, tietoturvaan sekä tehokkuuteen. Ensihoidon tehtävämäärät lisääntyvät, ja tarvitaan uusia järjestelmiä tukemaan hoito-ohjeprosessia. Yksi tärkeä osa tätä prosessia ovat konsultaatiot ja niiden kehittäminen. Videokonsultaatiolla voitaisiin mahdollisesti parantaa potilaan hoidon aloitusta sekä potilas voitaisiin tehokkaammin ohjata oikeaan palveluun tai hoitaa kotona. Researches show that there has been an increase in non-urgent calls for emergency care and in many cases the patients could have instructions to stay at home or to use some other, more adequate health care services than emergency clinic. The guideline process for care instructions includes evalua tion of the care needed, planning the care and the objectives. As a part of the care instruction regimen a paramedic often makes a consultation request for a doctor. This research aimed at investigating the possibilities and threats of video con sultation as a part of prehospital care regimen and to provide information to use when further developing the prehospital care regimen. Video consultation might have an impact on how the consultations succeed and the quality of care the patient will have. The research material was gathered as a qualitative inquiry research. The in quiry was implemented using social media to contact professional paramedics and it used the Webropol platform. Fifty persons answered the survey. The re search material was analysed using inductive content analysis. The paramedics that answered the inquiry thought that video consultation had potential to be used as a part of evaluating the care needed, as a support for communication, evaluating physical injuries and as a way to get advice for the treatment. Also, it could help to evaluate if the patient needed transport by an ambulance. The paramedics also impressed their thoughts about the prob lems of video consultation. The problems might include difficulties in learning how to use it, implementing, technical challenges, information security and ef ficiency. The need for paramedic missions is rising and new systems are needed to support the process of creating regimen. Making the consultations more effi cient is a part of the process. A video consultation might improve the speed for starting the treatments, create better ways to refer the patient to the right health care services or create ways to treat the patient at home.
