Hoidon tarpeen arvioinnin kehittäminen Lasten ja nuorten päivystyksessä : ohjekortit triageen

Abstract

Hoidon tarpeen ja kiireellisyyden arvioinnista käytetään sanaa triage. Triagen tavoite on edistää päivystyksen tehokkuutta ja turvallisuutta, ohjaamalla potilaat oikeaan hoitoon ja hoitolinjaan. Suomessa triagehoitajilla ei ole valtakunnallisia osaamiskriteerejä, mutta hoitajan tulee olla riittävän kokenut. Triagessa toteutetaan potilaan tehokas tilanarvio mahdollisimman vähin resurssein.

Tämän opinnäytetyön toimeksiantajana on TYKS:n Lasten ja nuorten klinikka ja aiheena on Lasten ja nuorten päivystyksen hoidon tarpeen kehittäminen. Opinnäytetyön tarkoituksena on luoda apukeino hoitohenkilökunnan päivittäiseen työhön ohjekorttien avulla. Tavoitteena on yhtenäistää päivystyksen toimintaa sekä potilaiden hoitoa. Hoidon tarpeen arviointia käsitellään hoidon kiireellisyyden arvioinnin, kiireellisyysluokkien sekä Lasten ja nuorten päivystyksen toiminnan kautta. Lisäksi työssä käsitellään lasten yleisimpiä oireita.

Opinnäytetyön tuotoksena luodaan tietopohja työntekijöille potilaan hoidon tarpeen arvioinnista sekä hoitolinjan valinnasta. Ohjekortit ovat sekä sähköisessä että fyysisessä muodossa.

Tuotos on helposti käytettävä sekä selkeä. Ohjekortteja voidaan hyödyntää apuna ja ohjeena hoidon tarpeen arvioinnissa ja hoitolinjan valinnassa. Lisäksi tämän työn tuotosta voidaan hyödyntää päivystyspoliklinikan triagen kehittämisessä sekä uusien työntekijöiden ja opiskelijoiden perehdyttämisessä. Jatkotutkimuksena voitaisiin selvittää potilaana olevan lapsen tai nuoren sekä perheen näkökulmaa hoitoprosessista ja päivystyspoliklinikan toiminnasta.

The assessment of the need for and urgency of treatment is referred to as triage. The goal of triage is to promote the efficiency and safety of emergency care, directing patients to the right treatment and line of care. In Finland, triage nurses do not have national competence criteria, but the nurse must be sufficiently experienced. In tirage, an efficient assessment of the patient´s condition is carried out with as few resources as possible.

This thesis is commissioned by the TYKS Department of paediatrics and adolescent medicine and the topic is developing the need for emergency care for children and young people. The purpose of the thesis is to create a tool for the daily work of nursing staff with the help of instruction cards. The aim is to harmonize emergency services and patient care. The assessment of the need for care is processed through the assessment of the urgency of care, urgency categories and the activities of the emergency services for children and young people. In addition, the thesis deals with the most common symptoms in children.

As a result of the thesis, a knowledge base is created for employees on assessing the patient´s need for treatment and choosing a treatment line. The instruction cards are both in electronic and physical form.

The output is easy to use and clear. The instruction cards can be used as an aid and guidance in assessing the need for treatment and choosing a treatment line. In addition, the results of this work can be used to develop triage in the emergency room and to provide orientation for new employees and students. Further research could investigate the perspective of the patient and the family on the treatment process and the operation of the emergency clinic.
