Saattohoidon osaamisen edistäminen keuhkosairauksien vuodeosastolla

Abstract

Väestön ikääntyessä kasvaa myös palliatiivisen hoidon ja saattohoidon tarve. Keuhkosairauksien vuodeosastoilla hoidetaan säännöllisesti kuolevia potilaita. Vakavat keuhkosairaudet voivat edetä saattohoitovaiheeseen nopeasti. Saattohoidon tarpeen kasvaessa on syytä kiinnittää huomiota myös hoitohenkilöstön osaamiseen ja hoidon laadun yhdenvertaisuuteen. Edellytyksenä laadukkaan saattohoidon toteuttamiselle on hoitohenkilöstön osaamisen vahvistaminen ja varmistaminen.

Tämän kehittämisprojektin tavoitteena oli edistää kohdeyksikön saattohoidon laatua ja saattohoito-osaamista yhtenäistämällä toimintatapoja, sekä vahvistamalla oikea-aikaista oirehoitoa. Kehittämisessä pidettiin erityisen tärkeänä käytännöstä ja hoitohenkilöstön näkökulmasta nousseita tarpeita. Osana kehittämisprojektia järjestettiin kohdeyksikössä työpaja, jonka tarkoituksena oli selvittää hoitohenkilöstön tärkeimmiksi kokemat kehittämisalueet- ja tarpeet saattohoitoon liittyen, sekä ideoida osana kehittämisprojektia luotua tuotosta.

Tämän kehittämisprojektin tuotoksena laadittiin tarkistuslista tukemaan saattohoidon yhteneväistä toteutusta kohdeyksikössä. Tarkistuslistan luomiseen hyödynnettiin työpajassa esille nousseita kohtia, sekä kirjallisuuskatsauksen teoriatietoa. Lisäksi toteutettiin vertailukehittämistä, jonka avulla saatiin tietoa yksiköistä, joissa vastaava tuotos on jo ollut käytössä. Tarkistuslista on suunniteltu käytännölliseksi hoitotyön apuvälineeksi. Se auttaa muistamisessa, tukee hoitokäytänteiden yhtenäistämisessä, sekä tukee laadukkaan saattohoidon toteuttamisessa keuhkosairaille potilaille.

As the population ages, the need for palliative care and end-of-life care is also increasing. Pulmonary diseases inpatient wards are regularly taking care of dying patients. Serious pulmonary illnesses can progress quickly into the end-of-life care phase. As the need for end-of-life care is growing, should the quality and equality of care also be considered. In order to provide high-quality end-of-life care, the competence and knowledge of the nursing staff must be endorsed and ensured.

The aim of this development project was to improve the practices and quality of end-of-life care in the target unit by unifying the practices, and to increase the know-how of the nursing staff providing end-of-life care. Acknowledging the needs of the nursing staff carrying out end-of-life care in practice, was a very important part of this project. A workshop was organised on a pulmonary ward to clarify those needs and to brainstorm a checklist that was created as part of the project.

As a result of this development project an end-of-life care checklist was created. The checklist is based on researched information and the needs of the nursing staff, which were discovered at the workshop. Benchmarking was also carried out as part of the project. Benchmarking provided information from the units that had already been using similar tools in practice. The checklist is a support tool for the nurses who are carrying out end-of-life care. The checklist will help the nursing staff to remember the steps of end-of-life care. It can also help to unify the practices and to carry out high quality end-of-life care for patients suffering from terminal stage lung diseases.
