Päiväkirurgisen tekonivelpotilaan hoitoprosessin kehittäminen Satasairaalaan- Potilaiden näkemykset, mielipiteet ja kehittämisehdotukset

Abstract

Päiväkirurgiset tekonivelleikkaukset ovat kasvussa, niitä on tehty Suomessa vuodesta 2019 lähtien. Satasairaalassa tehdään polven- ja lonkan tekonivelleikkauksia yli tuhat vuodessa. Päiväkirurgisia tekonivelleikkauksia ei ole vielä aloitettu. Väestön ikärakenne on muuttunut ja väestö tulee olemaan yhä vanhempaa. Tämä tulee lisäämään tekonivelien tarvetta tulevaisuudessa. Päiväkirurginen tekonivelleikkaus on osoittautunut turvalliseksi vaihtoehdoksi oikein valikoiduilla potilailla.

Tämän tutkimuksellisen kehittämistyön tarkoituksena oli kuvata polven- ja lonkan tekonivelleikkaukseen tulevien potilaiden mielipiteitä ja halukkuutta päiväkirurgisesti tehtävään tekonivelleikkaukseen. Tavoitteena oli selvittää, mitä sisältyy päiväkirurgisen tekonivelpotilaan hoitoprosessiin aikaisemman tutkimusnäytön mukaan ja tuottaa ehdotus päiväkirurgisen tekonivelpotilaan hoitoprosessista Satasairaalaan.

Tutkimukseen otettiin mukaan järjestyksessä potilaat, jotka olivat tulossa tekonivelleikkaukseen syksyn 2023 aikana ja täyttivät suosituksen mukaiset päiväkirurgisen potilaan kriteerit. Tutkimukseen otettiin yhteensä 30 polven tekonivelpotilasta ja 30 lonkan tekonivelpotilasta.

Tutkimuksen kyselyaineistoon vastasi yhteensä 35 potilasta ja näistä puhelinhaastatteluun vastasi 34. Vastanneista puolet (n=17) oli lonkan– ja puolet (n=17) polven tekonivelpotilaita. Tulokset osoittivat, että halukkuutta päiväkirurgiseen tekonivelleikkaukseen löytyi 37% ja 38% potilaista koki pystyvänsä kotiutumaan leikkauspäivänä. Luodussa hoitoprosessissa kuvataan mitä asioita kuuluu päiväkirurgisen tekonivelpotilaan hoitoprosessiin ennen leikkausta, leikkauksen aikana, leikkauksen jälkeen, kotiutuessa ja kotona.

Osa potilaista koki pystyvänsä kotiutumaan leikkauspäivänä, mutta monet kokivat yhden yön viettämisen sairaalassa turvalliseksi. Onnistuneen päiväkirurgisen tekonivelpotilaan hoitoprosessin luominen voi lisätä potilaan luottamusta ja turvallisuuden tunnetta päiväkirurgista tekonivelleikkausta kohtaan.

Amount of outpatient joint replacement surgery patients are increasing. In Finland they have been performed since 2019. At Satasairaala there are over one thousand joint replacement surgeries performed on knees and hips per year. Change on population structure and as the population ages the need for joint replacement surgery increases. Outpatient joint replacement surgery has been proven to be safe method on correctly chosen patients. Outpatient joint replacement surgeries are not yet performed at Satasairaala.

Intent of this research-based development study was to portray opinions and willingness to undergo outpatient joint replacement surgery on patients that were scheduled for knee or hip replacement surgery. Study’s objective was to clarify what is included on same day discharge joint replacement surgery patients nursing process according to earlier studies and to produce a proposal for outpatient joint replacement surgery process to Satasairaala

Study includes patients that were scheduled for joint replacement surgery during the fall of 2023 and fulfilled the requirements for outpatient surgery. 30 patients with knee replacement and 30 patients with hip replacement surgeries were selected to study by scheduling order.

35 patients answered the written questionnaire and out of those 35 patients 34 answered the follow-up phone interview. Half (n=17) of these went through knee replacement surgery and half (n=17) hip replacement surgery. The result indicates that 37% of patients were willing to go through outpatient joint replacement surgery and 38% felt like they could have been discharged from the hospital at the day of surgery. The created proposal will describe the nursing process before surgery, during surgery, after surgery, when discharged and at home.

Some of the patients felt like they could have been discharged from the hospital at the day of surgery but spending one night at the hospital instead of home was seen as a safer option. Successfully created nursing process might increase patient trust and the feeling of security towards outpatient joint replacement surgery.
