Tiivistelmä
The aim of the thesis was to bring attention to the outcomes of exercise-based physiotherapy treatment to those of operative treatment in the management of non-traumatic subacromial pain syndrome. The findings were used to enhance the clinical reasoning and patient education practices provided by the author’s peers. A thorough review of the literature was undertaken surrounding the topic for two main reasons.

The primary objective of this thesis was to conduct a systematized literature review and explore the outcomes of exercise therapy and subacromial decompression plus physiotherapy. This was achieved by drawing a direct comparison between the long-term (≥ 1 year) outcomes related to pain, function, and quality of life.

The second objective was to collect relevant literature surrounding the topic and provide a general understanding of subacromial pain syndrome as a phenomenon, familiarizing the reader with the assessment competency (diagnostic tools) and potential exercise strategies.

The thesis was implemented as a systematized literature review. Relevant articles related to the topic were systematically identified and collected between January 2024 and November 2024. The core theoretical framework was written between June and December 2024. The search utilized the databases of PubMed, ENBASE, and MEDLINE. In total five randomized control trials were selected for this systematized literature. Based on the PEDro scale, the selected studies can be considered of good methodological quality.

In conclusion, moderate to high levels of evidence suggest that the efficacy of surgery plus exercise therapy compared to exercise therapy alone on improving function and pain are negligible to be considered clinically important at the 12, 24, and 60-month follow-up.
No conclusion can be made regarding the 8-10-year follow-up outcomes. The available literature covering that time frame lacks adequate rigor and the sample size is too small. This further highlights the need for high-quality research focusing on the long-term (≥ 10 years) exploring the research question of this systematized literature review.
