Tiivistelmä
Background: Diabetes mellitus is one of the chronic conditions that affects humans regardless of age and it is associated with long-lasting complications. Different studies have established diabetes as a public health problem. Diabetic care is a burden globally. As the population grows older, the need for elderly homes and home care services is rising alongside a higher occurrence of diabetes. However, research increasingly shows that diabetes care in these care homes often falls short of recommended standards. The underlying causes of these issues are multifaceted and not well understood.
Task and objectives: To find out how the care of older people living in elderly homes or receiving home care services can be improved from existing scientific literature. The objective is to show different strategies meant to improve the nursing care of elderly diabetic people.
Method: The study utilized literature review method. Three scientific databases, CINAHL, Medline and Pub-Med, were used to find the data. Critical appraisal method was used in data selection process. The selected scientific literature materials were analyzed using the content analysis method.
Results: The improvement of nursing care for elderly people in elderly homes was categorized into three main categories. Patient-centered care, assessment and patient health education as the first category. The second category was a Multiprofessional collaboration where the nurses appreciated the roles of other professionals in the care of the elderly people since it enhanced the care and improved the general outcome of the disease progression. The third category was Organizational factors and guidelines related to implementation of nurs-ing intervention where the external factors limited the care provided by the nurses.
Conclusion: Nurses working with a vulnerable population such as the elderly should be equipped with enough resources to help improve the nursing care. Furthermore, the nurses should undergo trainings and be provided with guidelines that will assist them in the delivery of services. Collaboration with other professionals proved to be of benefit since there is better management of the disease which improves the general outcome of the patient’s health. More studies and research should be carried out in relation to the improvement of nursing care, especially among the elderly population. For example, are there any specific diabetic guidelines targeting multiprofessional teams in elderly care and how do they differ from other guidelines?
