Laboratorieundersökningar vid diagnostisering av akut lymfatisk leukemi

Abstract

Examensarbetet är en litteraturstudie med syfte om att redogöra vilka laboratorieundersökningar som tas vid misstanke om akut lymfatisk leukemi. Arbetet fokuserar på att ge en ökad förståelse för de olika typerna av prover och analyser som tas i samband med att sjukdomen utreds och diagnostiseras. I arbetet beskrivs även sjukdomen över lag, dess symptom, behandling samt hela laboratorieundersökningsprocessen. Frågeställningarna för arbetet är följande: Vilka laboratorieundersökningar tas vid misstanke om akut lymfatisk leukemi? Vad är syftet med dessa undersökningar? Vilken betydelse har molekylärgenetiska tester vid diagnostisering och klassificering av akut lymfatisk leukemi?

Akut lymfatisk leukemi (ALL) är den vanligaste cancerformen hos barn, och i Finland insjuknar årligen runt 50 barn. ALL är en elakartad form av blodcancer som uppstår på grund av genetiska förändringar i stamcellerna i benmärgen, vilket leder till att omogna vita blodkroppar, så kallade lymfoblaster, bildas okontrollerat. Syftet med arbetet är att bidra till ökad kunskap om ALL och redogöra vilka laboratorieundersökningar som tas för att diagnostisera sjukdomen.

Bland de första proverna som tas vid misstanke om ALL ingår en fullständig blodbild, differentialräkning samt blodutstryk, vilka ger en bild av blodstatus i kroppen och eventuell förekomst av omogna celler. Med hjälp av blodprover, benmärgsundersökning, immunfenotypning och genetiska tester kan diagnosen fastställas med hög säkerhet.

The thesis is a literature study with the aim of describing which laboratory tests are performed when Acute Lymphoblastic Leukemia (ALL) is suspected. The thesis focuses on providing an increased understanding of the different types of samples and analyses that are performed in connection with the investigation and diagnosis of the disease. It also provides information about the disease in general, its symptoms, treatment, and the entire laboratory testing process. The research questions are as follows: What laboratory tests are performed when Acute Lymphoblastic Leukemia is suspected? What is the purpose of these tests? What is the significance of molecular genetic tests in the diagnosis and classification of Acute Lymphoblastic Leukemia?

Acute Lymphoblastic Leukemia is the most common type of cancer in children, with approximately 50 cases diagnosed annually in Finland. ALL is a malign form of blood cancer caused by genetic changes in stem cells in the bone marrow, which leads to the uncontrolled formation of immature white blood cells, so-called lymphoblasts. The aim of this study is to enhance knowledge about ALL and to explain which laboratory tests are used to diagnose the disease.

Among the first tests taken when ALL is suspected are a complete blood count, differential count, and blood smear, which provide a picture of the blood status in the body and the possible presence of immature cells. With the help of blood tests, bone marrow examination, immunophenotyping and genetic tests, the diagnosis can be established with a high degree of certainty.
