Tiivistelmä
Väestön ikääntyminen on maailmanlaajuinen ilmiö. Tämän myötä geriatristen potilaiden hoidon osaamistaito korostuu ja sen myötä osastoille tarjottavan geriatrisen tuen tarjoamiselle on kasvava tarve. Tämän ylemmän ammattikorkeakoulun opinnäytetyön tavoitteena oli luoda materiaalia Varsinais-Suomen hyvinvointialueen (Varha) hoitokoordinaattoritoimintaan, erikoisairaanhoitoon suuntautuvaan geriatriseen konsultaatiotoimintaan.
Julkaistua materiaalia Suomessa toteutettavasta geriatrisesta konsultaatiotoiminnasta ei löytynyt, joten tässä työssä hyvinvointialueiden erikoissairaanhoidossa käytettävistä geriatrisista konsultaatio toimintamalleista tietoa kerättiin haastatteluilla benchmarkaten kolmea hyvinvointialuetta. Tavoitteena oli oppia jo toisten hyvinvointialueiden hyväksi todetuista malleista.
Haastatteluiden avulla saatiin kartoitettua kolmen eri hyvinvointialueen toimintamallit, ja näiden materiaalien pohjalta saatiin luotua toimintamallia Varha:n oman mallin luomiseen ja varmistusta myös jo etukäteen tehtyihin käytäntöihin osastokonsultaatioita varten. Population ageing is a global phenomenon. Therefore, the need for expertise in the care of geriatric patients becomes more prominent. Consequently, there is a growing demand for providing geriatric support to hospital wards. The aim of this master's thesis was to create material for the care coordinators in The Wellbeing Services County of Southwest Finland (Varha), specifically for geriatric consultation services oriented towards specialized medical care.
No published material was found on geriatric consultations implemented in Finland, so in this study, information on geriatric consultation models used in specialized healthcare in the wellbeing counties was collected through interviews by benchmarking three wellbeing counties. The aim was to learn from models that have already proven successful in other wellbeing counties.
With the help of the interviews, it was possible to map the three different operating models these wellbeing counties use. Based on this material, an operating model was created for Varha's own model. The material was also used to confirm practices already established for the ward consultations.
