Tiivistelmä
Suomessa säännöllisiä sosiaali- ja terveyspalveluja käyttäviä ikääntyneitä on yli 150 000 ja käyttäjämäärä tulee todennäköisesti kasvamaan tulevaisuudessa entisestään. Yhteiskunnan tavoitteena on tukea ikääntyneiden toimintakyvyn ja terveyden ylläpitoa mahdollisimman pitkälle vanhuuteen. Ikääntyneen suun terveys vaikuttaa yleisterveyteen sekä elämänlaatuun. Riski suun terveydentilan heikentymiseen lisääntyy sairastumisen ja toimintakyvyn laskun myötä esimerkiksi ikääntyneillä ja muistisairailla.
Tämän opinnäytetyön tavoitteena oli kehittää sairaanhoitajaopiskelijoiden tietämystä ikääntyneiden akuutin suun hoidon tarpeen arvioinnista. Tarkoituksena oli tuottaa sairaanhoitajaopiskelijoille ajankohtainen sähköinen opas ikääntyneiden akuutista suun hoidon tarpeen arvioinnista. Akuutilla suun hoidon tarpeella tarkoitettiin tässä opinnäytetyössä ikääntyneiden suussa esiintyviä haittoja tai ongelmia, jotka vaativat ammattilaisen toteuttamaa hammashoitoa ja aiheuttavat hoitamattomana elämänlaadun ja terveyden heikentymistä.
Opinnäytetyön kohderyhmänä olivat Metropolia Ammattikorkeakoulun sairaanhoitajaopiskelijat. Tarve oppaan toteuttamiselle syntyi, koska Metropolia Ammattikorkeakoulun sairaanhoitajaopiskelijoiden opetussuunnitelmaan ei kuulu juurikaan suunhoidon opetusta. Tutkimustuloksia sairaanhoitajien puutteellisesta suun hoidon toteuttamisesta löytyi myös kansainvälisellä tasolla.
Opinnäytetyö toteutettiin toiminnallisena kehittämistyönä. Kehittämisprosessin aikana luotiin tutkitun teoriatiedon avulla tietoperusta, jonka avulla määriteltiin opinnäytetyön tuotoksessa käytetyt teemat. Teemoiksi nousivat ien- ja limakalvo-ongelmat, ongelmat hammasproteesien kanssa, hoitamaton karies, ja ikääntyneille kaatumisien yhteydessä tapahtuvat hammastapaturmat. Myös kuivan suun tunnetta ja syljen määrän vähenemistä käsiteltiin. Tuotoksessa käsitellään suun hoidon teemojen lisäksi ikääntyneen suun terveyttä ja yleisterveyden välistä yhteyttä, ikääntyneen akuutin suun hoidon tarpeen arviointia, käytäntöä ohjeistavaa ikääntyneiden suunhoitoa ja akuuttien tilojen ehkäisyä.
Tuotosta voivat hyödyntää sairaanhoitajaopiskelijoiden lisäksi esimerkiksi suuhygienistiopiskelijat, Metropolia Ammattikorkeakoulun lehtorit ja muut ikääntyneiden kanssa työskentelevät ammattilaiset. Opinnäytetyön jatkokehitysideoiksi nousivat suun hoidon tarpeen arvioinnin ohjeiden tuottaminen myös muille erityisryhmille ja moniammatillinen yhteistyö sekä sairaanhoitajaopiskelijoiden että työelämän toimijoiden kanssa.
There are more than 150,000 elderly people who are using social and health care services regularly in Finland and the number of users will likely increase further in the future. The aim of our society is to support the maintenance of the functional capacity and health of the elderly as far as possible in old age. The elderly´s oral health affects general health and quality of life. The risk of impaired oral health may increase illness and disability, for example in elderly and patients with memory disorders.
The aim of this thesis was to increase the knowledge of the nursing students about the evaluation of the need for acute oral health care for the elderly. The purpose was to provide the nursing students with an up-to-date guide on the acute evaluation of the need for oral health care in the elderly. In this thesis the acute need for oral treatment meant the disadvantages or problems of the elderly in the mouth, which require oral health care by a professional and cause untreated deterioration of the quality of life and health.
The target group of this thesis were nursing students of the Metropolia University of Applied Sciences in Helsinki, Finland. The need for implementation of the guide was created because the curriculum of a nursing student at Metropolia University of Applied Sciences does not include much instruction on oral health. Research findings on nurses’ deficient knowledge on oral health care were also found at international level.
This thesis was carried out as functional development work. During the development process, a theory section was created with the help of the theoretical knowledge that was used to determine the themes used in the thesis's output. Topics included gingival and soft tissue problems, denture problems, untreated caries and dental accidents in the elderly. Also, the feeling of dry mouth and the decrease in the amount of saliva were discussed. In addition to the oral health care themes, the output addresses the relationship between elderly´s oral health and general health, the evaluation of the acute need for oral health care for the elderly, and the practice of oral health care and prevention of acute conditions in the elderly.
In addition to nursing students, for example, oral hygiene students, lecturers of Metropolia University of Applied Sciences and other professionals working with elderly people can benefit from the output. The further development ideas of this thesis were to produce guidelines for the evaluation of the need for oral health care for other special groups and for multiprofessional co-operation with both nursing students and professional health workers.
