Tiivistelmä
NEWS-pisteytys on vuonna 2012 Isossa-Britanniassa julkaistu aikaisen varoituksen pisteytysjärjestelmä. Sen käyttö perustuu kuuden fysiologisen suureen mittaamiseen ja pisteyttämiseen taulukon avulla. Saadun pistemäärän mukaan voidaan päätellä potilaan riskiluokka. NEWS-pisteytys tunnistaa riskipotilaat 87% tarkkuudella. Pisteet kertovat potilaan tilan heikkenemisestä jo silloin, kun hoitajan silmä ei sitä vielä huomaa.
NEWS-pisteytys on kehitetty tunnistamaan vuodeosastolla hoidettavien potilaiden tilan heikentyminen. Sen käyttöä on laajennettu mm. kotihoitoon, ensihoitoon ja päivystykseen. NEWS-pisteytys luo tavallaan yhteisen kielen eri toimijoiden välille. NEWS-pisteytyksen on todettu olevan luotettava, herkkä ja yksinkertainen mittari potilaan voinnin seurantaan.
Tämän opinnäytetyön tarkoituksena oli kuvata NEWS-pisteytyksen ja Medanets®-sovelluksen käyttöönottamista Jyväskylän yhteistoiminta-alueen terveyskeskuksen vuodeosastoilla. Kyselytutkimuksen avulla selvitettiin hoitohenkilökunnan ja lääkäreiden osaamista pisteytyksen käytössä ja sitä, miten sitä käytettiin hyödyksi työssä. Kyselyn avulla saatiin tietää myös pisteytyksen käyttöä edistäviä ja estäviä tekijöitä.
Osaamisen arvioi riittäväksi lähes puolet vastaajista ja saman verran arvioi, että osaaminen ei ollut riittävää. Pisteytystä käytettiin satunnaisesti ja kolmas osa ei käyttänyt sitä koskaan. Kun tavoitteena on käyttää sitä kaikille vuodeosastohoitoon tuleville potilailla, sitä käytti kaikille potilaille vain alle kymmenesosa vastaajista. Kuitenkin pisteytyksen käyttäminen koettiin enimmäkseen helpoksi tai melko helpoksi.
Pisteytyksen käyttöä edistäviä tekijöitä olivat koulutus, menetelmän helppous ja positiivinen asenne sen käyttämiseen. Estävinä tekijöinä esille tulivat resurssien ja tietämyksen puute sekä johtaminen. Nyt käyttöön on jo saatu pisteytyksen tekemisen helpottamiseksi laminoidut taskukortit. Medanets®-sovelluksen käyttöönottaminen pilottikokeiluna kahdella vuodeosastolla tulee huomattavasti helpottamaan pistelaskentaa, sillä sovellus laskee ne automaattisesti.
Lääkäreiden kyselyssä esille tuli, että osaaminen on riittävää ja pisteytyksen käyttäminen melko helppoa. Se koettiin myös hyödylliseksi potilaan hoidon arvioinnin ja suunnittelun kannalta. Toisaalta puolet vastanneista lääkäreistä ei käsitellyt pisteitä lääkärin kierrolla.
NEWS-pisteytyksellä on kuitenkin rajoituksensa. Hoitajien ei tulisi sokeasti luottaa pisteytysjärjestelmiin, vaan käyttävät niitä tukena hoitotyössä. Laadukas hoito ei voi perustua ainoastaan tiettyyn malliin tai pisteytykseen. NEWS-scoring, the early warning scoring system, is published in Great-Britain in 2012. It is based on measuring and scoring six physiological quantity and evaluating them by a certain chart. By the score you get, you can conclude patient’s risk category. NEWS-scoring recognizes the risk patients with 87% specificity. The scores tell about patient getting worse even when the eye of a nurse could not yet see it.
NEWS-scoring has been developed to recognize if the inpatients condition gets worse at the ward. It is used nowadays also in homecare, emergency care and in emergency rooms in hospitals. NEWS-scoring creates in a way a common language between different actors. NEWS-scoring have been found to be reliable, sensitive and a simple indicator to follow-up the patient’s condition.
The purpose of this thesis is to describe the introduction of NEWS-scoring and Medanets®-application in the hospital wards of Healthcare center of Jyväskylä co-operation district. By questionnaire study was found nursing staff´s and doctors’ competence in NEWS-scoring and the ways how they used NEWS-scoring at their work. Some conducive and prohibitive things about using the scoring was also found.
The competence was evaluated decent by almost the half of the respondents and the same number evaluated that the competence was not decent. The scoring was used occasionally and the third never used it. To all patient it was used only under one tenth of nursing staff, when the goal is to use it to every patient who is coming to ward. Although using NEWS-scoring was found mostly easy or quite easy to use.
The conductive things using the scoring was training, the method was easy to use and positive attitude to use it. As the prohibitive things came out the lack of resources, knowledge and also the management. Now the nursing staff has been given new laminated pocket cards to make it easier to do the scoring. Medanets®-application has been taken in use by two wards as a pilot study and it will make it substantially easier to make scoring because the application counts the score automatically.
The questionnaire study for doctors showed that the competence was decent and using the scoring was experienced quite easy. It was experienced also useful in evaluating and planning the patients care. On the other hand, the half of the doctors didn´t use the scoring when meeting the patients at the ward.
NEWS-scoring has however limitations. The nurses should not trust the scoring systems blindly but use them as support in nursing. High-quality nursing cannot be based on just one model or scoring.
