Tiivistelmä
Osteoporoosi eli luukato on yleinen, murtumia aiheuttava terveysongelma Suomessa. Hoitokustannusten kurissa pitämiseksi ja väestön toimintakyvyn tukemiseksi, on osteoporoosin hoidossa tärkeää sekä murtumien ennaltaehkäisy että niiden hoito tehokkaan ja oikein toteutetun lääke- ja muun hoidon avulla.
Potilaan myönteisyys ja sitoutuminen lääkehoitoon on merkityksellistä sekä potilaan elämän laadun että yhteiskunnan kustannusten näkökulmasta. Tästä huolimatta jopa puolet pitkäaikaislääkitystä käyttävistä henkilöistä, myös osteoporoosipotilaista, jättää lääkkeensä ottamatta. Lääkehoitoon sitoutumisen edistämisessä lääketiedon ohjauksella ja luotettavalla lääketiedolla on tärkeä merkitys. Sairaanhoitajat ovat suurin yksittäinen ammattiryhmä, joka on vastuussa lääkehoidon käytännön toteuttamisesta. Tämä opinnäytetyö on kuvaileva kirjallisuuskatsaus, jonka tarkoituksena oli selvittää osteoporoosipotilaiden lääkehoitoon sitoutumista ja siihen vaikuttavia tekijöitä. Tavoitteena oli tuottaa tietoa, joka lisää osteoporoosipotilaiden parissa työskentelevän hoitohenkilökunnan tietämystä sekä potilaiden sitoutumisesta osteoporoosin lääkehoitoon, että siihen vaikuttavista tekijöistä. Hoitohenkilökunnan paremman tietämyksen ansiosta potilaat saavat yksilöllisempää, heidän tilanteeseensa mahdollisimman hyvin soveltuvaa lääkehoidon ohjausta, mikä edistää lääkehoitoon sitoutumista.
Katsauksen tulosten perusteella voidaan sanoa, että potilaiden lääkehoitoon sitoutuminen oli pääsääntöisesti huonoa ja siihen vaikuttivat useat eri sisäiset ja ulkoiset tekijät, kuten potilaaseen, hoitohenkilökuntaan ja lääkkeeseen liittyvät tekijät. Tämän takia hoito ja siihen liittyvä potilasohjaus tulisi aina suunnitella ja toteuttaa potilaskohtaisesti ja yhteistyössä potilaan kanssa. Potilasohjauksen on oltava tarpeen tullen joustavaa, tutkittuun tieteeseen perustuvaa ja ajantasaista tietoa sisältävää, jotta hoitohenkilökunta pystyy tiedon lähteenä kilpailemaan muiden tietolähteiden kanssa. Osteoporosis, or bone loss, is a common health problem causing fractures in Finland. In order to keep the nursing costs under control and to support the physical functioning of the population, it is important to both prevent fractures and treat them by means of effective and properly implemented medication and other treatments.
The patient's adherence and commitment to medication are important both in terms of the quality of the patient's life and the cost to society. Despite this, up to half of people taking long-term medication, including patients with osteoporosis, do not take their medicine. Patient education and counselling and reliable medical information play an important role in promoting adherence to medication. Nurses are the largest single professional group responsible for the practical implementation of medical treatment. This thesis is a descriptive literature review aimed at investigating the adherence to pharmacotherapy among osteoporosis patients and factors promoting and preventing the treatment. The aim of the thesis was to increase the awareness of the nursing staff working with patients with osteoporosis about the adherence of patients to the medication and factors affecting it. As a result of better knowledge of the nursing staff, patients receive more individualized medication guidance that suits their situation as well as possible, thus contributing to better medication adherence.
Based on the results of this review, it can be said that the patient's adherence to medical treatment was generally poor and was affected by several internal and external factors such as patient, medical and nursing staff, and drug-related factors. For this reason, care and related patient counselling should always be planned and implemented on a patient basis and in cooperation with the patient. Patient counselling should be flexible and include up to date information based on the newest research information, so that medical and nursing staff can compete with other sources as a source of information.
