Tiivistelmä
Legg-Calvé-Perthesiin (LCP) sairastuu vuosittain noin 100 lasta Suomessa. LCP aiheuttaa kipua ja pysyviä luun muutoksia reisiluun päähän. LCP:n taudin kesto on yleensä noin kolmesta neljään vuotta. Tänä aikana lapsi ja hänen vanhempansa tapaavat säännöllisesti fysioterapeuttia sekä muuta henkilökuntaa moniammatillisesta tiimistä, joten ammattihenkilöillä tulee olla ajankohtaisin tieto opastaakseen perhettä kuntoutuksen edetessä. Uusien tutkimusten valossa ja entistä paremman diagnostiikan avulla yhä useampi LCP:tä sairastavista kyetään kuntouttamaan konservatiivisesti, jolloin vain kaikkein vakavimmat tai myöhään huomatut reisiluun pään nekroositapaukset joudutaan operoimaan. Fysioterapeutti tarvitsee aktiivisen, lasta ja vanhempia kuntouttamiseen kannustavan otteen työhönsä, jotta lapsi jaksaa pitkän kuntoutumisen ja vanhemmat sitoutuvat prosessiin.
Tämän työn tarkoituksena on systemaattista kirjallisuushakua käyttäen etsiä uusinta tutkimustietoa LCP:n kuntoutuksesta fysioterapeuttisin keinoin. Opinnäytetyö tehdään yhteistyössä HUS Uusi Lastensairaalan fysioterapian poliklinikan kanssa, ja tavoitteena on tuoda fysioterapeuteille ja muille terveydenhuollon ammattilaisille uusinta tutkimustietoa LCP:tä sairastavien lasten kuntoutuksesta.
Aineistoa haettiin neljästä eri tietokannasta talvella 2020: PubMed, PEDro, Scielo ja Semantic scholar. Kirjallisuuskatsaukseen valikoitui 11 tutkimusta, jotka käsittelivät LCP:n fysioterapiaa ja hoitoa.
Tutkimustulosten perusteella LCP:tä sairastavan lapsen fysioterapian intensiteetti ja sisältö määräytyvät yksilöllisesti lapsen iän, toimintakyvyn ja taudin vakavuuden perusteella. LCP:tä sairastavan lapsen fysioterapiassa arvioidaan säännöllisesti lapsen toimintakykyä. Fysioterapiaan kuuluu molempien lonkkien passiivinen ja aktiivinen liiketerapia, vartalon liikehoito, venyttely, tasapaino-, koordinaatio-, ja voima- ja kävelyharjoitteita sekä kotona tehtävät harjoitukset. Lisäksi perinteisen fysioterapian ohelle suositellaan allasterapiaa, joka voi lisätä liikeratoja enemmän kuin pelkät maalla tehtävät harjoitteet. Fysioterapian avulla voidaan ylläpitää ja kehittää lonkan liikkuvuutta, lonkan liikkeistä vastaavien lihasten lihasvoimia, reisiluun pään ja lonkkamaljan yhteneväisyyttä sekä lievittää kipua. Aktiivinen fysioterapia estää lonkan toimintahäiriöiden syntymistä ja pitkällä aikavälillä voidaan ennaltaehkäistä myös lonkan nivelrikon syntymistä. Legg-Calvé-Perthes disease (LCP) is diagnosed in 100 children per year in Finland. LCP causes pain and permanent changes in the femoral head. The duration of LCP is usually three to four years. A child with LCP and his/her parents regularly meet a physiotherapist and an orthopedist during those years, who needs to have the latest information so that they are able to guide the family during the rehabilitation process. More and more of the children with LCP will recover with the conservative treatments thanks to more detailed examination methods and earlier diagnosis, and only the most difficult cases will be treated with surgical operations. It is of outmost importance that the physiotherapist takes an active role and engages the child and the parents in the rehabilitation process. This is important to ensure that the child will stay motivated because the rehabilitation process is long.
The purpose of this thesis is to summarize the latest research of physiotherapy and the methods of rehabilitation in LCP. This thesis was made in co-operation with HUS New Children`s Hospital policlinic of physiotherapy and the goal was to produce current information of rehabilitation on children affected by LCP disease to professionals.
Four different databases were used which were PubMed, PEDro, Scielo and Semantic scholar. 11 studies that involved physiotherapy and other treatments in LCP were included this literary survey.
Based on the study, the intensity and content of physiotherapy for children affected by LCP disease is determined individually. The child`s age, functional capacity and the severity of the disease have a significant role in the choice of the methods of rehabilitation. In the physiotherapy for children with LCP, the functional capacity is estimated regularly. Physiotherapy consists of passive and active movement therapy for both hips, body movement therapy, stretching, balance, coordination, walking and strength exercises. In addition, the child also receives home exercises. Including aqua therapy with the more traditional physiotherapy should also be considered, considering that it can increase mobility more than floor exercises alone. With the help of physiotherapy, it is possible to maintain and develop hip mobility, muscle strength for hip movements, the coherence of the femoral head and acetabulum, and to relieve pain. Active physiotherapy prevents the development of hip dysfunction. In the long term the occurrence of hip osteoarthritis can also be prevented.
