Tiivistelmä
Kouluaikana lapset itsenäistyvät ja vastuu suun hoidosta siirtyy enenevissä määrin heille itselleen. Vastuun ollessa lapsella, terveystiedon opetukseen sisältyvän suunhoidon opetuksen merkitys korostuu kouluissa. Suun tervey-denhoito on osa kouluterveydenhuoltoa, ja tulee näin huomioida osana opetussuunnitelmaa. Koulun terveystiedon opetuksen lisäksi koululaiset saavat suun hoitoon tukea suun terveydenhuollolta. Suun terveydenhuollon on jär-jestettävä koululaisille suun terveystarkastukset ensimmäisellä, viidennellä ja kahdeksannella luokalla, ja välivuosi-tarkastuksia suuhygienistille tarvittaessa. Koululaisten suun terveystarkastuksia järjestettäessä, suun terveyden-huollon ja koulun tiiviin yhteistyön merkitys kasvaa.
Opinnäytetyö on kehittämistyö ja sen aiheena on 5-luokkalaisten suun terveystarkastukset Non Stop -toimintamallilla. Non Stop -toimintamallissa oppilaille ei anneta yksilöllistä tarkastusaikaa hammaslääkärille, vaan opettaja lähettää oppilaat luokasta suun terveydenhuollon vastaanotolle vuorotellen satunnaisessa järjestyksessä. Kehittämistyön tavoitteena on tehostaa alakoululaisten suun terveystarkastusten sujuvuutta ja yhtenäistää toimintatapoja alakoululaisten suun terveystarkastuksissa PHHYKY:n alueella. Tavoitteena on myös kehittää yhteistyötä suun terveydenhuollon ja opettajien välillä. Tarkoituksena on tuottaa Non Stop -toimintamalli yhteistyössä PHHY-KY:n kanssa ja kokeilla toimintamallia suun terveydenhuollon vastaanotolla Lahden alueella. Tarkoituksena on ke-rätä palautetta Non Stop -toimintamallin kokeilusta ja palautteen pohjalta muokata toimintamalli sellaiseksi, että se voidaan ottaa käyttöön PHHYKYn alueella. Tarkoituksena on myös kuvata Non Stop -toimintamalli prosessikaavioon ja kirjoittaa tiedote suun terveystarkastuksista, joka voidaan lähettää Wilman kautta koululaisten huoltajille.
Opinnäytetyössä raportoinnissa käytettiin lineearisen kehittämisen mallia. Prosessien kuvaamisessa otettiin huomi-oon suun terveydenhuollon ammattilasten näkemyksiä, sekä kokemuksia koululaisten suun terveystarkastuksista. Tiedotteen teossa hyödynnettiin teoriapohjaa ja ammattilasten näkemyksiä hyvästä tiedotteesta. Toimintamallia kokeiltiin 5-luokkalaisten suun terveystarkastuksissa.
Toimintamallin toimivuutta arvioitiin ryhmähaastattelusta ja yksilöhaastatteluista saatujen palautteiden perusteella. Haastateltavina oli hammaslääkäri, hammashoitaja, kokeiluun osallistuneiden 5-luokkalaisten opettajat sekä viisi huoltajaa. Prosessin kuvausta pidettiin palautteen perusteella hyvänä. Pieniä aikataulumuutoksia kuitenkin toivot-tiin toimintamalliin. Wilman kautta huoltajille lähtevää tiedotetta pidettiin selkeänä ja helposti ymmärrettävänä.
Prosessikaavioita ja tiedotekirjettä muokattiin sopiviksi palautteiden perusteella. Toimintamallin prosessikuvauksen avulla se voidaan jalkauttaa muihin suun terveydenhuollon vastaanottoihin PHHYKYn alueella. Toimintamallia voidaan hyödyntää myös muiden alakoulun luokkien suun terveystarkastuksissa. Tiedote soveltuu huoltajille lähetettäväksi kaikissa alakoulun suun terveystarkastuksissa. During school, children become independent and the responsibility for oral care increasingly shifts to them. With responsibility for the child, the importance of oral care instruction in health education is emphasized in schools. Oral health care is part of school health care and is therefore considered as part of the curriculum. In addition to teaching school health information, pupils receive support for oral care from oral health care. Oral health care must arrange oral health examinations for pupils in the first, fifth and eighth grades, as well as intermediate ex-aminations for the oral hygienist if necessary. When organizing oral health examinations for pupils, the importance of close cooperation between oral health care and the school is growing.
The thesis is a development work and its topic is oral health examinations for 5th graders with the Non Stop oper-ating model. In the Non Stop operating model, students are not given an individual dental examination time to the dentist, but the teacher sends students from class to the dental clinic in a random order.The aim of the develop-ment work is to enhance the fluency and to harmonize the operating methods of oral health examinations for pri-mary school children in the PHHYKY area. The aim is also to develop cooperation between oral health care and teachers. The aim is to produce a Non Stop operating model in cooperation with PHHYKY and to try out the oper-ating model in a dental clinic in the Lahti region. The purpose is to collect feedback for the experiment with the Non Stop operating model and, on the basis of the feedback, modify the operating model so that it can be intro-duced to the PHHYKY area. It is also intended to describe the Non Stop operating model in a process diagram and to write an bulletin on oral health examinations, which can be sent through Wilma to guardians.
The model of linear development was used for reporting in the thesis. The descriptions of the processes took the views of oral health professionals into account, as well as the experiences of schoolchildren's oral health examina-tions. When writing the bulletin, the theoretical basis was used and the views of professionals on a good bulletin were considered. The operating model was tested during oral health examinations of 5th graders.
The effectiveness of the operating model was assessed on the basis of feedback from group and individual inter-views. The interviewees included a dentist, a dental nurse, teachers of the 5th graders who participated in the experiment, and five caregivers. The description of the process was considered good based on the feedback. However, minor schedule changes were desired to the operating model. The bulletin sent to the guardians through Wilma was considered clear and easy to understand.
The process diagrams and the bulletin were modified as appropriate based on the feedback. With the help of the process description of the operating model, it can be implemented in other dental clinics in the PHHYKY area. The operating model can also be utilized in oral health examinations of other primary school classes. The bulletin is suitable for sending to guardians at all primary school oral health examinations.
