Tiivistelmä
Opinnäytetyö oli toiminnallinen kehittämistyö, jonka tilaajana oli Kuopion kaupungin terveydenhuollon palvelualue, aikuisten kuntoutuspalvelut, aikuisten avotiimi. Teimme tutkimustiedon pohjalta oppaan vatsalihasten erkaantuman palautumiseen sekä äidin hyvinvoinnin tukemiseen. Tavoitteena oli lisätä tietoa vatsalihasten erkaantumasta ja sen kuntoutumisesta synnyttäneille äideille. Lisäksi opas toimii apuna fysioterapeuteille, jotka työskentelevät vatsalihasten erkaantuma-asiakkaiden kanssa.
Opinnäytetyön tekeminen aloitettiin kesällä 2019 tutkimustiedon keräämisellä ja rajaamisella sekä kohderyhmän valinnalla. Teoriaosuus saatiin valmiiksi tammikuussa 2020, jonka jälkeen lähdimme hahmottelemaan Eroon erkaantumasta omahoito-opasta synnyttäneille naisille. Opas saatiin koetestaukseen huhtikuussa 2020, jonka jälkeen pystyimme tekemään oppaaseen viimeiset muutokset ennen sen virallista käyttöönottoa. Toukokuussa 2020 viimeistelimme opinnäytetyön.
Vatsalihasten erkaantumasta puhutaan, kun suorien vatsalihasten välinen rako on yli 2,5 cm. Erkaantuma voi esiintyä missä kohtaa vain linea albaa vatsalihasten välissä rintalastasta häpyluuhun. Raskauden viimeisen kolmanneksen aikana erkaantumaa esiintyy melkein kaikilla naisilla ja synnytyksen jälkeen erkaantumaa on vielä jopa 60 % naisista. Tutkimukset ovat osoittaneet, että vatsalihasten erkaantuman esiintyvyyteen, kokoon ja sen palautumiseen voi vaikuttaa säännöllisellä harjoittelulla ja liikunnalla. Tutkimuksissa ei ole todettu tiettyä harjoittelumuotoa, joka ehkäisisi vatsalihasten erkaantumaa. Kuntoutumisessa tulee huomioida, että jokainen on yksilö ja palautuminen tapahtuu omaan tahtiin. Vatsalihasten erkaantuman palautumiseen on harjoitteita, joissa yhdistyvät hengitys, syvienlihasten aktivointi, liike ja rentoutus.
Oppaaseen valituilla harjoitteilla on ollut merkitystä vatsalihasten erkaantuman palautumiseen ja keskivartalon hallitumpaan toimintakykyyn. Synnyttäneiden äitien hyvinvointi koostuu useista eri tekijöistä, joilla on merkitystä kehon palautumiseen synnytyksen jälkeen. Opas tule olemaan apuna fysioterapeuttien työskentelyssä erkaantuma asiakkaiden kanssa. Lisäksi se tulee olemaan synnyttäneiden äitien käytössä. The thesis was a functional development work and the client organisation was the City of Kuopio: the rehabilitation services for adults in the Kuopio city health care service area, adult open team. Based on the research data, the authors made a guide to the recovery from abdominal muscle separation and to the well-being of mothers. The aim was to increase knowledge about abdominal muscle separation and its rehabilitation for mothers who have given birth. In addition, the guide serves as an aid to physiotherapists working with abdominal muscle clients.
The thesis was started in the summer of 2019 with the collection and delimitation of research data and the selection of the target group. The theoretical part was completed in January 2020, after which the authors started to outline the self-care guide for women who have given birth, in order to guide them to get rid of the separation. The guide was piloted in April 2020, after which the final changes were made and the guide officially introduced. In May 2020, the thesis was completed.
The term abdominal muscle separation (diastasis recti) its used when the gap between the straight abdominal mus-cles is more than 2.5 cm. Abdominal muscle separation can occur anywhere between the abdominal muscles from the sternum to the pubic bone. During the last trimester of pregnancy, diastasis recti occurs in almost all women, and after childbirth, diastasis recti is still present in up to 60% of women. Studies have shown that the incidence, size, and recovery of abdominal muscle separation can be influenced by regular exercise and sport. Studies have not identified any specific form of training that would prevent abdominal muscle separation. Rehabilitation should take into account that everyone is an individual and recovery takes place at their own pace. There are exercises that combine breathing, deep muscle activation, movement and relaxation to restore abdominal muscle separation.
The exercises selected into the guide have been important for the recovery of abdominal muscle separation and for more controlled functioning of the middle body. The well-being of mothers who have given birth consists of several different factors that are important for the recovery of the body after childbirth. The guide will help physiotherapists work with diastasis recti clients. In addition, it will be for the independent use of mothers who have given birth.
