Tiivistelmä
Infektoiden ehkäisy on osa potilasturvallisuutta. Hoitoon liittyvien infektioiden torjunnassa yksi merkittävin keino on oikein toteutettu käsihygienia. Siksi käsihygienian osaaminen on keskeinen työmenetelmä terveydenhuollossa. Käsihy-gienian toteuttamisesta on tehty paljon tutkimuksia sekä laadittu ohjeita, mutta siitä huolimatta käsihygieniassa on vielä paljon puutteita terveysalalla. Tämän vuoksi tietoisuuden lisääminen käsihygienian merkityksestä on tärkeää.
Opinnäytetyön tuotos oli opetusvideo käsihygienian oppimisen tehostamiseen hyödyntämällä Objective structured video examination (OSVE) menetelmää. OSVE tarkoittaa arviointimenetelmää, jossa opiskelijoiden oppimista arvioidaan vide-oon perustuvien kysymysten avulla. Videon tavoitteena on edistää opiskelijoiden käsihygienian osaamista lisäämällä heidän tietoperustaansa ja tiedon soveltamista käytännön hoitotyöhön. Työn tilaaja oli Savonia- ammattikorkeakoulu ja InovSafeCare hanke, jossa Savonia on mukana. Hankkeessa kehitetään Portugalin, Espanjan, Puolan ja Suomen yhteis-työnä infektioiden torjunnan koulutusta terveysalalle.
Opinnäytetyössä laadittu opetusvideo perustuu ajankohtaiseen kirjallisuuteen sekä tutkimuksiin ja se on tehty hoitotyön opettajien ohjauksessa. Opetusvideolla kuvataan perifeerinen kanylointi käsihygienian ja aseptiikan näkökulmasta. Kanylointi teknisenä toimenpiteenä ei ole videolla pääasia, vaan tärkein opetuksellinen asia on käsihygieniaosaaminen.
Opetusvideo pilotoitiin neljällä hoitotyönopiskelijalla, joilta saadun palautteen perusteella se viimeisteltiin. Palautteen mukaan opetusvideo oli selkeä ja se eteni loogisesti. Käsihygienian merkitys tuli videolla hyvin esille. Opetusvideo luovutetaan Savonia-ammattikorkeakoululle ja InovSafeCare hankkeelle opetuskäyttöön. Savonia saa muokata edel-leen tuotettua videota ja käyttää sitä haluamallaan tavalla erilaisissa opetuksen tilanteissa. Video liitetään osaksi InovSafeCare hankkeen verkko-oppimisympäristöä.
Käsihygienian osaamisen kehittäminen on tulevaisuudessakin tärkeää, sillä siinä on vielä paljon puutteita. Luomalla uusia opetusmenetelmiä sekä kehittämällä olemassa olevia, voidaan käsihygienia opetusta tehostaa sekä vahvistaa opiskelijoiden aikaisempaa tietoa asiasta. Jatkossa käsihygienian edistämiseksi tulee säännöllisesti kouluttaa terveys-alan ammattilaisia ja opiskelijoita käsihygienian merkityksestä hoitoon liittyvien infektioiden torjumisessa. Infection prevention is part of patient safety. One of the most significant means of combating treatment-related infections is properly implemented hand hygiene. Therefore, competence in hand hygiene is a key working met-hod in healthcare. A lot of research has been done and many guidelines have been made on the implementation of hand hygiene, but there are still many shortcomings in hand hygiene in the health sector. Therefore, raising awareness of the importance of hand hygiene is important.
The purpose of the thesis was to produce an instructional video to enhance the learning of hand hygiene by utilizing the Objective structured video examination (OSVE) method. OSVE refers to an assessment method in which student learning is assessed using video-based questions. The aim of the video is to promote students' hand hygiene skills by increasing their knowledge base and applying the knowledge to practical nursing work. The client organisation of the thesis was Savonia University of Applied Sciences, and this thesis is part of the InovSafeCare project, in which Savonia is involved. The project is developing co-operation between Portugal, Spain, Poland and Finland in infection control training for the health sector.
The teaching video prepared in the thesis is based on current literature and research and has been made under the guidance of nursing teachers. The instructional video describes peripheral cannulation from the perspective of hand hygiene and asepsis. Cannulation as a technical measure is not the main thing in the video, but the most impor-tant educational thing is hand hygiene skills.
The instructional video was piloted by five nursing students, after which it was finalized, based on the feedback received from the students. According to the feedback, the instructional video was clear and progressed logically. The importance of hand hygiene came to the fore in the video. The instructional video will be handed over to Savonia University of Applied Sciences and the InovSafeCare project for educational use. Savonia is allowed to further edit the video produced and use it as they wish in different teaching situations. The video will be integrated into the e-learning environment of the InovSafeCare project.
The development of hand hygiene skills will continue to be important in the future, as there are still many shortcomings. By creating new teaching methods and developing existing ones, hand hygiene teaching can be made more efficient and students' previous knowledge of the subject can be strengthened. In the future, in order to promote hand hygiene, health professionals and students should be regularly trained on the role of hand hygiene in combating treatment-related infections.
