Tiivistelmä
Kohdunkaulan syöpä on maailmanlaajuisesti naisten neljänneksi yleisin syöpä. Kohdunkaulan syövän seulonnat aloitettiin gynekologisella irtosolukokeella (papa-näyte) 1960-luvulla Suomessa. Tehokkaan seulonnan ansiosta kohdunkaulansyöpä on Suomessa nykyisin naisten 19. yleisin syöpä. Tutkimukset ovat kiistatta osoittaneet, että ihmisen papilloomaviruksen (HPV) aiheuttama pitkittynyt infektio aiheuttaa kohdunkaulan syöpää. Terveyden ja hyvinvoinnin laitos suositti vuonna 2011, että HPV- testi otetaan käyttöön 30–60- vuotiaiden naisten kohdunkaulan syövän seulonnassa ja 20–25- vuotiaiden seulontatestinä käytetään edelleen perinteistä papa-näytettä. Kansallisen asiantuntijaryhmän antaman suosituksen mukaan Suomessa seulonnassa käytetään vain validoituja HPV- testejä, jotka osoittavat korkean riskin (high risk) HPV- tyypit.
Tämän opinnäytetyön toimeksiantaja on Savonia Ammattikorkeakoulu. Tämän kehittämistyön tarkoituksena oli koota sähköinen oppimateriaali ihmisen papilloomaviruksesta, HPV- testistä ja uudistuvasta kohdunkaulansyövän seulonnasta Suomessa. Kehittämistyön tavoitteena oli kehittää terveydenhuoltoalan opiskelijoiden tietoisuutta kohdunkaulan syövän seulonnasta itsenäisen opiskelun avulla.
Kehittämistyötä varten haettiin artikkeleita, jotka sisälsivät tutkimustietoa HPV- testin käytöstä rinnakkain papa- näytteen kanssa kohdunkaulan syövän seulonnassa Suomessa. Tietoa haettiin myös kohdunkaulan syövästä, seulonnasta, papilloomaviruksesta, kolmesta seulontakäyttöön hyväksytystä HPV- testistä, tutkimusmenetelmien periaatteista, sekä hyvästä oppimateriaalista. Kehittämistyön tuloksena koottu oppimateriaali antaa selkeän käsityksen nykyisestä kohdunkaulan syövän seulonnasta ja HPV- testin käytöstä tutkimusmenetelmänä Suomessa. Oppimateriaali tehtiin sähköiseen muotoon ja se liitetään Moodleen kliininen histologia ja sytologia kurssille. Oppimateriaali on muokattavissa suositusten muuttuessa tai uusia HPV- testejä otettaessa käyttöön. Cervical cancer is the fourth most common cancer among women worldwide. Cervical cancer screenings with Pap smear began in Finland in the 1960s. Thanks to effective screening, cervical cancer is now the 19th most common cancer among women in Finland. Scientific research has clearly shown that prolonged infection caused by human papillomavirus (HPV) is the cause of cervical cancer. In 2011, the Department of Health and Welfare recommended that the HPV test is used for cervical cancer screening for women aged 30 to 60 and a conventional Pap smear will continue to be used as a screening test for women aged 20 to 25. The national scientific group of cervical cancer screening in Finland gave their recommendation of the validated high risk HPV tests.
The client of this thesis was Savonia University of Applied Sciences. The purpose of this development work was to produce digital learning material on human papillomavirus, HPV testing and evolving cervical cancer screening in Finland. The aim of the development work was to develop the awareness of healthcare students about cervical cancer screening in Finland through independent study.
Articles containing research data on the use of the HPV test in co-testing with a conventional pap smear in cervical cancer screening in Finland were sought for the development work. Data retrieval was carried out on cervical cancer screening, papillomavirus, three HPV tests approved for screening and new research methods of the HPV- tests, as well as good learning material. The product of the development work was a learning material that provides a clear understanding of the current cervical cancer screening and the use of the HPV test as a new testing method. The learning material was made in digital form and it is attached to Moodle's clinical histology and cytology course. It is easy to edit the learning material when recommendations change or when new HPV tests are introduced.
