Näkemättä liikkeelle : näkövammaisen lapsen varhaisvaiheen motoriikan tukeminen

Abstract

Tämän opinnäytetyön tavoite oli koota suomeksi tietoa pienen näkövammaisen lapsen fysioterapian erityispiirteistä. Tarkoituksena oli tehdä monimuotoinen opinnäytetyö, jonka teoreettista osaa voivat hyödyntää näkövammaisten lasten kanssa työskentelevät fysioterapeutit. Opinnäytetyötä voi lisäksi hyödyntää moniammatillisessa yhteistyössä näkövammaisen lapsen motorisen kehityksen tukemisessa. Liitteenä olevat käsittelykuvat ovat käyttöön ohjatessa lapsen vanhempia ja lähipiiriä lapsen motorista kehitystä tukevaan arkeen. Motorisen kehityksen teoriaksi työssä on Neuronal Group Selection Therory. Teoria kuvaa normaaliin motoriseen kehitykseen kuuluvaa vaihtelua, joka ei ole satunnaista vaan perustuu sekä geneettiseen tietoon että hermoverkkojen valikoitumiseen. Valikoitumiseen vaikuttaa suuresti afferentti tieto ja motorinen käyttäytyminen. Tuloksena on tutkittuun tietoon perustuva kooste näkövammaisen lapsen motoriikan erityispiirteistä ja aistien hyödyntämisestä motoriikan varhaisvaiheen tukemisessa. Oikaisu- ja tasapianoreaktioiden ja painonsiirron kehittyminen ovat oleellisia näkövammaisen lapsen kehityksessä. Ne mahdollistavat asennosta toiseen siirtymisen ja itsenäisen liikkumisen. Tuotos on teoriaosan tutkittuun tietoon perustuva kuvapankki varhaisvaiheen motoriikan tukemiseksi. Valokuvissa kuvataan otteita ja asentoja, joita käytetään fysioterapiassa näkövammaista lasta fasilitoidessa. Kuvia hyödynnetään vanhempien ohjauksessa työpaikoillamme. Osa kuvista on teoriaosassa, muut opinnäytetyön tekijöiden käytössä. Opinnäytetyöstä saadaan tärkeää tietoa, sillä suomenkielistä vastaavaa työtä ei ole aikaisemmin tehty. Jatkossa hyödyllisenä tutkimuksen kohteena voisi olla aistien merkitys pienten lasten fysioterapiassa.

The purpose of this practical thesis was to gather knowledge in Finnish about the characteristics of physiotherapy concerning infants with visual impairment. The aim of the theoretical part was to collect information which can be used by physiotherapists as well as other professionals when working with visually impaired infants in order to promote their motor development. The motor development theory used is the Neuronal Group Selection Theory. The theory states that human motor development is characterized by a variation that is not random but determined by epigenetic information and neuronal group selection. The selection is based on afferent information produced by behavior and motor experience, thus sensory information has an important function in motor development. Atypical motor development is described by how it affects variation, i.e. the size of repertories and variability, i.e. the ability to select effective motor strategies. We collected evidence based information about the most common motor problems associated with visual impairment as well as information on how to promote sensory mapping of all the senses to motor development. The development of righting and equilibrium reactions and weight shift is the critical point in motor development of visually impaired small infants and toddlers, because they enable movement transition and locomotion, both of which are difficult for infants with visual impairment. This is why adequate instructions are important for parents handling these children. The functional part consists of photos to be used when instructing parents and significant others in handling that promote the child’s motor development. The aim was to take pictures that show clearly how to facilitate movement and handle the infant in daily activities. Some of the photos are included in the theoretical part, whereas most of them will only be in restricted use by authors. The outcome is important as there is no similar collected information in the Finnish language about how to promote motor development and practical handling of visually impaired children. An important future study could be of the significance of the calibration of sensory inputs onto motor development.
