Elvytyksen jälkeinen hoito – itseopiskelumateriaali: Opetusvideo ensihoitajaopiskelijoille

Abstract

Potilaan tila vaatii elvytyksen jälkeen välittömiä toimenpiteitä. Ensivaiheen toimista käytetään nimitystä postresuskitaatiohoito eli elvytyksen jälkeinen hoito. Sairaalan ulkopuolella tapahtuneis-sa sydänpysähdyksissä ensihoidon rooli on merkittävä. Tehokkaalla ja oikein kohdennetulla hoi-dolla potilaan elintoiminnot saadaan vakautettua ja lisävaurioiden synty estettyä. Alustava elot-tomuuden työdiagnoosi lyhentää hoitoviiveitä sairaalan sisällä ja siten myös parantaa potilaan toipumisennustetta.

Teimme opinnäytetyönämme opetusvideon elvytyksen jälkeisestä hoidosta Oulun ammattikor-keakoululle. Videolla käydään läpi potilaan ROSC:n jälkeiset tutkiminen ja toimenpiteet ABCDE-protokollan mukaan, 4H4T-muistisääntö ja ei-tekniset taidot sekä konsultaatio osana ROSC:n jälkeistä hoitoa. Videolla aihe käsitellään näyteltynä potilastapauksena, johon on liitetty teoriaa diojen ja äänitteiden muodossa.

Tarve opetusvideolle elvytyksen jälkeisestä hoidosta syntyi opinnäytetyön tekijöiden omasta aloitteesta, sillä kokemamme mukaan itseopiskelumateriaali kyseisestä aiheesta oli tarpeellinen. Projektimme tarkoituksena oli tuottaa laadukas ja selkeä opetusmateriaali Oulun ammattikorkea-koulun ensihoidon opiskelijoiden ja opettajien käyttöön. Pitkän aikavälin kehitystavoitteena on parantaa valmistuneiden ensihoitajien tietotaitoa ja osaamista ROSC:n jälkeisestä hoidosta siten, että he voivat hyödyntää näitä taitoja työelämään siirtyessään.

Opinnäytetyötä varten keräsimme tietoperustaa elvytyksen jälkeisestä hoidosta, ei-teknisistä taidoista tiimityöskentelyssä sekä oppimisesta ja video-opetusmateriaaleista. Lähteinä käytimme laadukkaita suomalaisia ja kansainvälisiä lähteitä. Näistä tärkein on elvytyksen Käypä hoito -suositus, johon videon teoriaosuudet perustuvat. Video tuotettiin täysin omakustanteisesti, ja tilaajana toimi Oulun ammattikorkeakoulu. Videota tuottaessamme keräsimme palautetta sekä ensihoidon opettajilta että toisen ja kolmannen vuoden ensihoitajaopiskelijoilta. Palautteilla var-mistetiin tuotteen laatu ja soveltuvuus opetusvideoksi. Valmis tuotos luovutetaan opetuskäyttöön Oulun ammattikorkeakoululle.

After a successful resuscitation following a cardiac arrest a patient’s condition requires immediate procedures. The term used for the initial procedures is post resuscitation care, meaning the pro-cedures needed to be done after the return of spontaneous circulation. The role of the emergency care services is significant in post cardiac arrest care situations outside a hospital. With efficient and correctly targeted treatment the vital signs of a patient can be stabilized, and further damag-es prevented. Prelimary diagnosis about the reason for the cardiac arrest shortens delays inside a hospital and therefore also increases the patient’s chance for recovery.

As our thesis we produced an educational video about post resuscitation care for the Oulu Uni-versity of Applied Sciences (OUAS). The video shows examining the patient and procedures according to the ABCDE protocol, the 4H4T memory rule, non-technical skills and consulting a doctor as a part of the post resuscitation care treatment. In the video the subject is presented as a dramatized patient case with included theory. The included theory appears in the form of slides and voice overs.

Need for the educational video originated from the makers’ own initiative. Based on our own ex-periences self-study material about post cardiac arrest care was necessary. The goal of our pro-ject was to develop a high-quality and clear study material which the students and teachers at OUAS can use. The long-term development goal is to improve future paramedics’ knowledge and skills concerning post resuscitation care so that they can apply these skills when they transition to working life after graduation.

For the thesis we gathered knowledge about post resuscitation care, non-technical skills in teamwork, learning and video study material. We used high quality both Finnish and international sources. The most important source was the Finnish Current Care Guidelines (Käypä hoito) on which the theory parts of the thesis are based upon. The video was subscribed by OUAS and it was completely produced at our own cost. While we were producing the video, we collected feedback from the second- and third-year emergency care students. We used the feedback to ensure the quality and usability of the video as educational material. The finished product will be handed over for utilization as educational material to OUAS.
