Tiivistelmä
Tämän tutkimuksen tarkoituksena oli löytää keinoja, miten motivoida asukas lääkehoitoon palvelutaloyksikössä. Tutkimusongelmana havaittiin asukkaan kielteisyys sekä kysymys, onko hoitajilla riittävästi tietoa lääkehoidosta. Tutkimuksessa yritettiin löytää haittatekijöitä sekä keinoja asukkaan motivoitumiseen lääkehoidossa. Tutkimuksen tilaajana toimi Vaasan Validian asumispalveluyksikkö. Tutkimus toteutettiin kvalitatiivisella eli laadullisella tutkimusmenetelmällä. Vaasan Validian yksikössä tutkimukseen osallistui 10 hoitajaa. Hoitajille annettiin vastausaikaa kolme viikkoa. Tutkimus toteutettiin sähköisellä e-lomakkeella.
Tutkimuksessa keskeisinä käsitteinä käytettiin lääkehoitoa, lääkehoitoon sitoutumattomuutta, johon sisältyi lääkevastaisuus, sekä lääkeluvallista hoitajaa, palvelutalon asukasta ja motivointia. Tietoa haettiin Finnasta, Terveyskirjastosta, Terveysportista sekä Medicin tietokannoista. Tutkittaessa ilmeni, että lääkevastaisuus on yleinen haittatekijä lääkehoitoon sitoutumisessa. Siksi lääkevastaisuus käsiteltiin lisänä lääkehoitoon sitoutumattomuudessa. Lääkehoito käsitteenä tiivistettiin tutkimukseen, koska tutkimuksessa yritettiin löytää keinoja lääkehoidon motivoimiseen.
Tutkimuksen keskeisenä tuloksena havaittiin hoitajan vuorovaikutuksen tärkeys asukkaan kanssa. Vuorovaikutustapoja on monia. Toimivan vuorovaikutusmenetelmän löytäminen asukkaalle pohjautuu keskinäiseen luottamukseen hoitosuhteessa. Hoitajan ammatillisen osaamisen jatkuva kehitys tuo mahdollisuuksia asukkaan lääkityksen motivointiin. Potilas ei pysty motivoitumaan lääkehoitoon ilman selkeää tietoa hoidon hyödyistä ollessaan oireeton. Potilas epäilee lääkkeiden sivuvaikutuksia sekä tarvetta syödä useampaa lääkettä. WHO:n aiemmassa tutkimuksessa havaittiin, että vain puolet lääkehoidosta toimii suunnitellusti, vaikka lääkäreiden mielestä lääkehoito toteutuu paremmin. Joskus potilas ei muista lääkkeen ottoa. The purpose of this study was to find ways to motivate the resident in pharmacotherapy in a sheltered housing unit. The research problem was the resident's non-compliance to pharmacotherapy and the question of whether nurses have enough information about pharmacotherapy. The aim of the study was to find ways to motivate the resident for pharmacotherapy. The client organisation was the sheltered housing unit of Validia in Vaasa. The study was carried out with a qualitative research method. The study was participated by 10 nurses. The response time was three weeks. The study was conducted using an electronic e-form.
The key concepts of the study are pharmacotherapy, pharmacotherapy non-compliance which included medication non-adherence, and a nurse with a medical license, a resident of a residential unit and motivation. Information was searched from Finna, Terveyskirjasto, Terveysportti and Medic database. The study revealed that pharmacotherapy non-compliance is a common adverse factor in adherence to pharmacotherapy. That is why the concept of pharmacotherapy non-compliance was explained in addition to medication non-adherence. Pharmacotherapy as a concept was also explained in the study because the goal was to find ways to motivate the patient to pharmacotherapy.
The most important tool of motivation is the nurse’s interaction with the resident. There are many ways to interact. Finding a functional method of interaction for the resident is based on mutual trust in the care relationship. The continuous development of the nurse's professional competence brings opportunities to motivate the resident's medication. The patient is not able to motivate themselves to pharmacotherapy without a clear information about the benefits of the treatment when the patient is not experiencing any symptoms. The patient suspects side effects of the medication and the necessity to take multiple medicines. A previous WHO study found that only half of pharmacotherapy works as planned, although doctors believe that pharmacotherapy works better. Sometimes the patient does not remember to take the medicine.
