Tiivistelmä
Neulatorakosenteesi on ensihoitajien tekemä henkeä pelastava toimenpide jänniteilmarinnan hoitoon sairaalan ulkopuolella. Jänniteilmarinta on henkeä uhkaava terveydentila, jossa keuhkopussiin kertyy ilmaa ja keuhkopussiin kertyvä ilma alkaa painamaan keuhkoa kasaan. Tilanteen kehittyessä ilma alkaa painamaan verisuonia ja sydäntä kasaan. Hoitamattomana tilanne johtaa verenkiertosokkiin ja kuolemaan. Hätätoimenpiteenä suoritettava neulatorakosenteesi tasaa keuhkopussiin kertyneen paineen, jotta verenkierto pääsee palautumaan. Tällä hetkellä suositellaan, että toimenpide tehdään suurella 5 cm laskimokanyylilla ja pistokohta on 2.–3. kylkiluunvälistä keskisolislinjassa.
Asiantuntijoiden keskuudessa on alkanut heräämään huolia neulatoraskosenteesin tehokkuudesta ja turvallisuudesta nykyisillä toimenpideohjeilla. Suomenkielistä tutkimusta tai materiaalia on hyvin niukasti saatavilla, joten tämän opinnäytetyön tarkoituksena on tuottaa tietoa toimenpiteen turvallisuudesta, mahdollisista komplikaatioista sairaalan ulkopuolisessa ensihoidossa. Tavoitteena oli koota aiheesta kattava suomenkielinen kokonaisuus.
Tämä opinnäytetyö on kuvaileva kirjallisuuskatsaus neulatorakosenteesin potilasturvallisuudesta sairaalan ulkopuolisessa ensihoidossa. Suoritin katsauksen tiedonhaun viiteen eri tietokantaan: Medic, Ebscon CINAHL, Pubmed ja Cochrane. Tietokannoista tuli hakutuloksena yhteensä 96 artikkelia, joista tähän katsaukseen valikoitui yhteensä kahdeksan tutkimusartikkelia. Hakulausekkeet olivat: ”(ilmarin* OR jänniteilmarin* OR paineilmarin*) AND (neulatorakosenteesi* OR neulatorakos-tomia*) AND (ensihoi* OR sairaalan ulkopuol*)”. Kansainvälisiin tietokantoihin hakulauseke oli ”(”tension pneumothorax” OR “pneumothorax”) AND (”needle thoracocentesis” OR ”need-le thoracostomy” OR ”needle decompression” OR “needle chest decompression”) AND (”pa-ramedic” OR ”emergency treatment” OR “emergency care” OR “prehospital” OR “pre hospital” OR “pre-hospital” OR “ambulance” OR “out of hospital”)”.
Katsauksen tuloksista selvisi, että neulatorakosenteesin epäonnistumisprosentti on korkeimmillaan 80 % luokkaa. Toimenpiteessä onnistumiseen vaikuttaa eniten neulan pituus, pistokohta ja rintakehän paksuus. Samoilla asioilla on myös vaikutusta toimenpiteen turvallisuuteen ja toimenpiteellä voidaan pahimmillaan aiheuttaa vakavaa haittaa potilaalle. Katsauksen tulokset viittaavat kuitenkin siihen, että vakavia komplikaatioita sattuu harvoin, mutta toimenpiteen epäonnistuminen on itsessään vakava komplikaatio, kun kyseessä on henkeä uhkaavan tilanteen hoitokeino. Tällainen vakava komplikaatio on myös potilasturvallisuusuhka, joten siihen pitäisi tulla muutosta. Needle thoracocenteses are performed by paramedics as life-saving procedure in the management of the tension pneumothorax. This life-threatening condition develops when air gets trapped abnormally in the pleural cavity through a one-way valve. As a result, air accumulates and compresses the lung, increases intrathoracic pressure enough to decrease venous return to the heart and eventually causing obstructive shock, which leads untreated to death. Tension pneumothoraxes are usually encountered in trauma and critical care environments.
In Finland, current emergency care literature recommends that the needle thoracocentesis is to be performed with 5 cm angiocatheter at the 2nd intercostal space midclavicular line, but recent international literature has been raising questions about the effectiveness of the procedure with 5 cm angiocatheter at the 2nd intercostal space midclavicular line. The aim of this thesis was to determine, is the needle thoracocentesis safe procedure to patients, performed by paramedics in prehospital setting. The aim was also to gather knowledge about possible complications of needle thoracocentesis.
This thesis is a narrative literature review that presents knowledge about needle thoracocentesis safety and possible complications related to the procedure. The literature search was done in five different databases: Finna.fi, Medic, PubMed, Ebsco CINAHL and Cochrane. The following search strategy was used in search ”(”tension pneumothorax” OR “pneumothorax”) AND (”needle thoracocentesis” OR ”needle thoracostomy” OR ”needle decompression” OR “needle chest decompression”) AND (”paramedic” OR ”emergency treatment” OR “emergency care” OR “prehospital” OR “pre hospital” OR “pre-hospital” OR “ambulance” OR “out of hospital”)”. Search resulted 96 articles in which eight were selected into this review.
The results show that most common complication of the procedure is that it fails quite often. Performing needle thoracocentesis with 5 cm angiocatheter at the 2nd intercostal space midclavicular line can have failure rate high as 82%. The results also designate that the success of the procedure is most affected by the length of the needle, the injection site and the chest wall thickness. Furthermore, the results show that severe complications happen only occasionally, but since lethality of untreated tension pneumothorax, that high failure rate is severe complication thus serious patient safety issue that needs to change.
