Tiivistelmä
Lantionpohjan toimintahäiriöt ovat yleisiä, hyvälaatuisia, mutta elämänlaatua heikentäviä vaivoja. Lantionpohjan toimintahäiriöihin luetaan virtsan, ulosteen ja suolikaasujen karkailu, lantionelinten laskeumat, seksuaalitoimintojen häiriöt, lantionalueen kivut ja jännitystilat, joiden vaikutus heijastuu koko kehoon. Raskaus ja alatiesynnytys ovat riskitekijöitä lantionpohjan toimintahäiriöille aiheuttamansa mekaanisen paineen ja kudosvaurioiden vuoksi.
Opinnäytetyön tarkoituksena oli selvittää lantionpohjan toimintahäiriöiden esiintyvyyttä alateitse synnyttäneillä naisilla, sekä äitiysneuvolan antamaa lantionpohjan lihasharjoitteluohjauksen laatua. Tutkimuksen kohderyhmänä olivat alateitse synnyttäneet naiset. Opinnäytetyön tavoitteena oli tuottaa uutta tietoa terveydenhoitotyön ammattilaisille siitä, miten äitiysneuvolan asiakkaat kokivat saamansa lantionpohjan lihasharjoittelun ohjauksen.
Tutkimusongelmilla haettiin vastauksia, millaisia lantionpohjanlihasten toimintahäiriöitä synnyttäneet äidit ovat kokeneet sekä millaista ohjausta ja tukea äidit ovat saaneet lantionpohjanlihasten toimintahäiriöihin ja keneltä. Opinnäytetyö toteutettiin määrällisen tutkimusmenetelmän mukaisesti. Aineistonkeruumenetelmänä käytettiin strukturoitua kyselylomaketta. Tutkimuksen aineisto kerättiin verkkokyselynä, joka jaettiin sähköpostin välityksellä, sekä Facebookissa erilaisiin vauva- ja perheryhmiin. Kyselyyn osallistujat saivat vastata aineistoon anonyymisti. Kyselytutkimukseen vastasi N=650 vastaajaa.
Tutkimus osoitti, että selkeästi yleisimmät oireet synnyttäneillä naisilla olivat virtsankarkailu, selkäkipu, niska-hartiakivut sekä virtsan pidättämisen vaikeus. Tutkimuksessa kävi myös ilmi, että suurin osa äideistä oli saanut vain kirjallista materiaalia lantionpohjanlihasten harjoittamiseen. Tutkimukseen vastaajista kuitenkin reilu kolmannes ei ole saanut ohjausta ja tukea lantionpohjanlihasten toimintahäiriöihin neuvolan kautta. Tutkimukseen osallistujat toivoivat neuvolakäyntien yhteyteen yhtä äitiysfysioterapiakäyntiä. Pelvic floor dysfunctions are common. They are usually benign and mainly female ailments which decrease the quality of a woman’s life. Pelvic floor disorders consist of faecal, urinary and gas incontinence, pelvic organ prolapse, sexual dysfunctions, chronic pelvic pain, and hypertonic pelvic floor disorders. Pregnancy, vaginal delivery and ageing are the main risk factors in developing these disorders due to the increased intra-abdominal pressure, direct muscle trauma, nerve injury and connective tissue damage.
The purpose of this thesis was to examine pelvic floor disorders amongst women who have given birth by vaginal delivery. The purpose was also to examine the quality of pelvic floor muscle training guidance the maternity clinics had given during pregnancy and after childbirth. The aim of this thesis was to produce new information to public health care professionals about how the clients of maternity clinic experienced the guidance they were given about pelvic floor exercise.
Research problems were created to examine the prevalence and the quantity of pelvic floor dysfunctions among women with history of at least one vaginal birth and the diversity of guidance and support for pelvic floor disorders they had experienced from health care professionals. The study participants were women who had given birth to a child by vaginal delivery. The inquiry of the thesis was executed with quantitative research method and the material was gathered by an electronic questionnaire which was distributed through e-mail and Facebook groups. The inquiry was answered by a total of 650 respondents.
The research data showed that the most generic symptoms respondents had experienced were urinary incontinence, neck, shoulder and back pain, and furthermore, involuntary leakage of urine during physical exertion. The results also showed that the majority of the mothers were only given leaflets to guide pelvic floor muscle training while one third of the respondents did not get any guidance at all. The participants to the inquiry wished that pelvic floor physiotherapy would be included in the maternity clinic’s scheduled check-up programme.
