Tiivistelmä
Opinnäytetyö toteutettiin kehittämistyönä yhteistyössä Kuopion yliopistollisen sairaalan (KYS) keuhkopoliklinikan kanssa. Opinnäytetyön tarkoituksena oli suunnitella, toteuttaa ja arvioida keuhkopoliklinikan CPAP-laitehoitoa käyttäville uniapneapotilaille potilasopas. Oppaan tavoitteena on lisätä uniapneapotilaiden hoitoon sitoutumista ja hoidon tärkeyden ymmärrystä sekä täydentää keuhkopoliklinikan työntekijöiden antamaa suullista potilasohjausta. Keuhkopoliklinikan uniapneapotilaat saavat oppaan mukaansa CPAP-hoidon ohjauskäynniltä, jolloin he voivat palata ohjauksessa käytyyn asiaan omalla ajallaan.
Uniapnea on sairaus, joka ahtauttaa hengitysteitä unen aikana ja aiheuttaa hengityskatkoksia. Uniapnean sairastavuus on lisääntynyt huomattavasti ja Suomessa vähintään lievää uniapneaa arvioidaan sairastavan 1,46 miljoonan ihmisen. Uniapnea nimetäänkin jo kansansairaudeksi. Uniapnean yleisin riskitekijä on ylipaino. Uniapnean liitännäissairauksia ovat tyypin 2 diabetes, iskeminen (hapen puutteeseen liittyvä) sydänsairaus, aivohalvaus, sydämen vajaatoiminta, rytmihäiriöt, verenpainetauti ja masennus.
Uniapnean hoitoon sitoutuminen on usein haasteellista, minkä vuoksi potilasohjaus on tärkeässä roolissa. Potilasohjaus on prosessi, jossa terveydenhuollon henkilöstö kannustaa potilasta ottamaan vastuuta omasta terveydestään, löytämään omia voimavaroja ja hoitamaan itseään hyvin. Potilasopas tukee terveydenhuollon henkilökunnan potilasohjausta. Ohjauksen sisältö jää todennäköisesti paremmin mieleen, kun suullinen ohjaus täydennetään kirjallisella ohjeella.
Aineisto kerättiin erilaisista tieteellisistä artikkeleista ja Käypä hoito -suosituksista. Tiedonhaussa on käytetty ammattikirjallisuus- ja nettilähteitä. Oppaan sisällössä on tietoa uniapneasta sairautena, uniapnean vaikutuksesta ajoterveyteen sekä omahoidosta ja sen merkityksestä. Oppaan sisällöstä sovittiin yhdessä KYS:n keuhkopoliklinikan työntekijöiden kanssa.
Jatkokehittämisideana on selvittää, onko potilasopas lisännyt omahoitoon sitoutumista ja ovatko potilaat saaneet oppaasta tarvitsemaansa tietoa omahoitoon liittyen. Potilasoppaan sisältöä voisi muokata potilaiden tarpeiden mukaisesti. Potilasoppaan voisi kääntää myös englanniksi, jotta sen hyödynnettävyys olisi laajempi potilaille, jotka eivät puhu suomea. The thesis was carried out as development work in collaboration with the Pulmonary Outpatient Clinic of Kuopio University Hospital (KUH). The purpose of the thesis was to design, implement and evaluate a patient guide for sleep apnea patients in the Pulmonary Outpatient Clinic. The aim of the guide is to increase the commitment to the treatment of sleep apnea patients and to understand the importance of the treatment and supplement the oral patient guidance provided by the staff of Pulmonary Outpatient Clinic. Sleep apnea patients at the Pulmonary Outpatient Clinic will receive the guide from the CPAP treatment visit, allowing them to return to the issue of the guidance on their own time.
Sleep apnea is disease that narrows the airways during sleep and causes interruption in breathing. Morbidity of sleep apnea has increased considerably, and it is estimated that in Finland there are 1,46 million people who have at least mild sleep apnea. Sleep apnea is already called a public disease. The most common risk factor of sleep apnea is overweight. Associated diseases of sleep apnea are type 2 diabetes, ischemic heart disease, stroke, heart failure, arrhythmias, hypertension, and depression.
Commitment to care of sleep apnea is often challenging, which is why patient education plays important role. Patient education is a process in which healthcare staff encourage patients to take responsibility for their own health, find their own resources and take good care of themselves. Patient guide supports the patient education of health care staff. The content of the education is better remembered when oral education is supplemented with written instruction.
The material has been collected from various scientific articles and current care recommendations. Professional literature and online sources have been used in the information retrieval. The content of the guide contains information about sleep apnea as disease, the effect of sleep apnea on driving health, and self-care and its importance. The content of the guide was agreed together with the employees of the KUH Pulmonary Clinic.
The idea of further development is to find out whether the patient guide has increased the commitment to self-care and whether patients have received the information they need regarding self-care from the guide. The contents of the patient guide could be adapted to the needs of the patients. The patient guide would also be translated into English so that its usability would be wider for patients who do not speak Finnish.
