Oxylog®-ventilaattorin käyttö non-invasiivisessa hengityshoidossa : perehdytysvideot Pohjois-Savon sairaanhoitopiirin ensihoitajille

Abstract

Opinnäytetyönä tehtiin kolme perehdytysvideota NIV- (noninvasiivinen ventilaatio) hoidosta. Videot käsittelivät Dräger Oxylog® VE300 – ventilaattorin osia, käyttöönottoa, NIV-hoidon aiheita sekä vasta-aiheita, NIV-hoidon aloitusta, potilaan monitorointia hoidon aikana sekä hoidon jatkamista, inhaloitavien lääkkeiden antoa NIV-hoidon aikana sekä yleisimpiä ongelmia ja niiden ratkaisuja hoidon aikana. Toimeksiantajana opinnäytetyössä oli Pohjois-Savon sairaanhoitopiirin (PSSHP) ensihoitopalvelut.

NIV-hoidolla tarkoitetaan hengityksen tukemista ventilaattorilla maskin avulla, ilman keinoilmatietä. NIV-hoitoa annetaan akuutissa hengitysvajauksessa. NIV-hoito aloitetaan akuutissa tilanteessa jo sairaalan ulkopuolella ensihoidossa ja ventilaattori kuuluukin ambulanssin varusteluun PSSHP:n ensihoitoyksiköissä. Ensihoitajalla pitää olla riittävä tietotaito sekä osaaminen ventilaattorin käytöstä sekä NIV-hoidosta. Tämä takaa sen, että ensihoitaja pystyy toteuttamaan hoidon itsenäisesti ja potilasturvallisesti.

Oxylog® VE300 – ventilaattorit löytyvät vuonna 2022 jokaisesta PSSHP:n ensihoitoyksiköstä, joten videoiden tarve uusien työntekijöiden perehdytyksessä sekä vanhojen työntekijöiden osaamisen ylläpitämisessä on il-meinen. Ventilaattoria käytetään usein työparin kanssa kahdestaan, sekä NIV-hoito aloitetaan itsenäisesti, eikä lääkärin konsultaatiota aina tehdä ensin. Videoiden tarkoituksena on, että työntekijät voivat omatoimisesti videoiden avulla kerrata laitteen toimintaa ja hoidon aloitusta, joka lisää hoidon varmuutta ja näin parantaa potilasturvallisuutta.

Jatkokehittämistyönä voisi kartoittaa NIV-hoidon osaaminen PSSHP:n alueen ensihoitajilla, tehdä perehdytys-materiaalit Oxylog® VE300 – ventilaattorin invasiivisesta ventilaatiohoidosta, sekä perehdytysvideoita pystyisi varmasti hyödyntämään myös ensihoitajaopiskelijoiden opetuksessa.

The topic of this thesis is NIV (non-invasive ventilation) treatment. The output of this thesis was three orienta-tion videos. The videos cover parts of Dräger Oxylog® VE300 – ventilator, introduction of ventilator, indications and contraindications for NIV-treatment, beginning of NIV-treatment, monitoring of the patient during the treatment, continuation of treatment, administration of inhaled drugs during the NIV-treatment and most com-mon problems during treatment and solutions to them. The client organisation for his thesis was emergency medical service of Hospital District of Northern Savo (PSSHP) NIV-treatment means supporting the breathing with a ventilator using a mask without artificial airway. NIV-treatment is given during acute respiratory failure. NIV-treatment is started by prehospital emergency care in acute situation and the ventilator is found among the equipment of ambulances in PSSHP. A paramedic is sup-posed to have enough knowledge and know-how about the usage of the ventilator and about the NIV-treatment. This guarantees that the paramedic can carry out the treatment independently and considering pa-tient safety. Oxylog® VE300 – ventilators are found on every ambulance of PSSHP in 2022 so need for videos to orientate new employees and to maintain the knowledge of old employees is apparent. A ventilator is often used alone with co-worker and the NIV-treatment is started independently without a doctor’s consultation first in many cases. The purpose of the videos is that the employees can independently go through the usage of the device and initiation of treatment. This adds certainty of treatment, and this way improves patient safety. A topic for further study could be an analysis of the know-how of paramedics in Hospital District of Northern Savo about NIV-treatment. Do an orientation material of Oxylog® VE300 – ventilator invasive ventilator treat-ment. And the orientation videos could also be used in the education of paramedic students.
