Yövuoron ajankäytön jakautuminen kliinisen kemian laboratoriossa

Abstract

Sairaalalaboratorion täytyy pysyä toiminnassa kellon ympäri vuoden jokaisena päivänä. Yövuoron aikana suoritettavat tutkimuspyynnöt toteutetaan päivystyspyyntöinä. Yövuorossa on huomattavasti vähemmän henkilöstöä aamu- ja päivävuoroon verrattuna. Yövuorossa ei esimerkiksi ole erikseen näytteenottajaa, jolloin analytiikasta vastaava henkilökunta huolehtii myös päivystyksenä tehtävistä tutkimuspyynnöistä.

Opinnäytetyö toteutettiin laadullisena tutkimuksena, joka suoritettiin osallistuvalla havainnoinnilla. Opinnäytetyössä tutkittiin HUS Diagnostiikkakeskuksen Etelä-Karjalan sairaalan kliinisen kemian laboratorion yövuoron sujuvuutta. Tutkimuksessa seurattiin yövuorossa työskentelevää kahta bioanalyytikkoa vuorotellen niin, että neljän peräkkäisen yövuoron aikana suoritettiin seurantaa sairaalan osastoille näytteenottoon lähtevän laboratoriohoitajan toiminnasta sekä laboratorioon jäävän laboratoriohoitajan toiminnasta. Neljästä peräkkäisestä yövuorosta ensimmäisenä ja kolmantena yönä seurattiin näytteenottoon lähtevää laboratoriohoitajaa, toisena ja neljäntenä yönä laboratorioon jäävää laboratoriohoitajaa. Neljä yövuoroa toteutettiin niin, että saatiin seurattua sekä arkiöitä, että yleensä kiireellisiksi koettuja viikonloppuöitä.

Opinnäytetyön tarkoituksena oli selvittää, miten yövuoron ajankäyttö jakautuu kliinisen kemian laboratoriossa vuoron aikana sekä kuinka hoitoyksiköiden tutkimuspyynnöt vaikuttavat työnkulkuun. Opinnäytetyössä kerättiin tutkimusaineistoa havainnoimalla ja strukturoimattomien haastattelujen avulla. Näin menettelemällä saatiin tietoa sekä työn kulusta ja kuormittavuuteen vaikuttavista tekijöistä. Opinnäytetyön tavoitteena oli tuottaa tietoa siitä, voidaanko kliinisen kemian laboratorion yövuorojen toimintaa kehittää ja kuormittavuutta vähentää prosessia muuttamalla.

A hospital laboratory must remain operational around the clock, every day of the year. Research requests during the night shift are carried out as emergency requests. The night shift has significantly fewer personnel compared to the morning shifts and day shifts. For example, there is no separate personnel to take samples on the night shift in which case the staff responsible for analytics also takes care of research requests made on duty.

This thesis was conducted as a qualitative study through participant observation. In the thesis, the fluency of the night shift in the clinical chemistry laboratory of the HUS Diagnostic Center of South Karelia Central Hospital was investigated. In the study, two medical laboratory technologists working on the night shift were monitored alternately, so that during four consecutive night shifts, the activity of the medical laboratory technologist who went to the hospital wards for sampling and the activity of the medical laboratory technologist who remained in the laboratory was monitored. On the first and third night of the four consecutive night shifts, the medical laboratory technologist who left for sampling was followed, and on the second and fourth night the medical laboratory technologist who stayed in the laboratory was monitored. The four night shifts were selected so that both weekdays and weekend nights, which are generally perceived as urgent, could be monitored.

The purpose of the thesis was to find out how the time use of the night shift is distributed in the clinical chemistry laboratory during the shift and how research requests from treatment units affect the workflow. In the thesis, research material was collected through observation and unstructured interviews. By proceeding in this way, information was obtained about the flow of work and the factors influencing the workload. The aim of the thesis was to provide information on whether the operation of night shifts in the clinical chemistry laboratory can be developed and the workload reduced by changing the process.
