Tiivistelmä
Ikääntyessä monilääkitys ja riskit lääkkeiden haitta- ja yhteisvaikutuksille kasvavat. Ikääntyneet ovat hyvin herkkiä lääkityksessä tapahtuville muutoksille. On tärkeää, että hoitajilla on tietoa ja osaamista ikääntyvien lääkehoidon erityispiirteistä sekä lääkehoidon toteuttaminen on selkeää ja turvallista.
Opinnäytetyö oli kehittämistyö, jonka tarkoituksena oli laatia Aurinkokartanoon lääkehoidon toteutusohje sähköiseen kansioon. Toimeksiantaja oli Pohjois-Savon hyvinvointialueen muistisairaiden asumispalveluyksikkö Aurinkokartano Iisalmessa. Yksikössä muutettiin lääkehoidon toteutus keskitetyksi tapahtuen jatkossa lääkehuoneessa. Muutosten myötä oli tarpeen päivittää lääkehoidon toteutusohjeet ajan tasalle.
Kehittämistyön tavoitteena on lääkehoidon turvallisuuden parantuminen Aurinkokartanossa. Lääkehoidon toteutusohjeen tavoitteena on yhtenäistää lääkehoidon toteuttamisen käytänteitä Pohjois-Savon hyvinvointialueen asumispalveluissa ja olla apuna Aurinkokartanon lääkehoitoon osallistuvan henkilökunnan perehdytyksessä.
Kehittämistyö toteutettiin konstruktivistisen mallin mukaan ja siinä laadittiin sähköinen toteutusohje lääkehoidon toteutukseen Aurinkokartanossa. Toteutusohjeessa käytiin läpi iäkkäiden lääkehoidon erityispiirteet, lääkehoidon turvallinen toteutus sekä lääkehoidon vaara- ja haittatapahtumat. Toteutusohjeen sisältö perustui lääkehoitoa ja sen toteutusta ohjaaviin lakeihin ja asetuksiin, lääkehoitosuunnitelmaan sekä tutkimustietoon ikääntyvien lääkehoidon erityispiirteistä. Toteutusohje tehtiin PowerPoint-pohjalle ja se liitettiin Aurinkokartanon Teams-sovelluksen sähköiseen kansioon.
Toteutusohje on Aurinkokartanon henkilökunnan ja myös uusien työntekijöiden helposti löydettävissä ja luettavissa. Aurinkokartanolla on oikeus päivittää toteutusohjetta, kun lääkehoitoon tulee muutoksia. Ajantasaiset ja yhteneväiset toimintatavat lisäävät myös ikääntyneiden lääkehoidon turvallisuutta. Jatkotutkimuksena voisi olla, miten keskitetty lääkehoito sekä sähköinen lääkehoidon toteutusohje ovat vaikuttaneet lääkehoidon turvallisuuteen Aurinkokartanossa. Abstract
As people age, polypharmacy and the risks of adverse drug reactions and interactions increase. Older people are very sensitive to changes in medication. It is important that nurses have knowledge and competence about the special features of pharmacotherapy for the elderly, and that the implementation of pharmacotherapy is clear and safe.
The thesis was a development work, the purpose of which was to draw up instructions for the implementation of pharmacotherapy in an electronic folder for Aurinkokartano. The client was the housing service unit Aurinko for people with memory disorders in Iisalmi in the wellbeing services county of North Savo. In the unit, the implementation of pharmacotherapy was changed to a centralised one, and in the future, it will take place in the medicine room. As a result of the changes, it was necessary to update the guidelines for the implementation of pharmacotherapy.
The aim of the development work was to improve the safety of pharmacotherapy at Aurinkokartano. The aim of the guidelines for the implementation of pharmacotherapy was to harmonise the practices of implementing pharmacotherapy in the housing services of the North Savo wellbeing services county and to assist in the familiarisation of the personnel involved in pharmacotherapy at Aurinkokartano.
The development work was carried out according to a constructivist model, and an electronic implementation guideline for the implementation of pharmacotherapy was drawn up at Aurinkokartano. The implementation instructions reviewed the special features of pharmacotherapy for the elderly, the safe implementation of pharmacotherapy, and the hazards and adverse events of pharmacotherapy. The content of the implementation guidelines was based on the laws and decrees governing pharmacotherapy and its implementation, the pharmacotherapy plan, and research data on the special characteristics of pharmacotherapy for the elderly. The implementation instructions were made on the PowerPoint template, and it was connected to the electronic folder of the Aurinkokartano Teams application.
The implementation instructions are easy to find and read by the Aurinkokartano staff and by new employees. Aurinkokartano has the right to update the implementation instructions when changes to medical treatment are made. Up-to-date and consistent operating methods also increase the safety of pharmacotherapy for the elderly. Further research could be on how centralised pharmacotherapy and the electronic guidelines for the implementation of pharmacotherapy will have affected the safety of pharmacotherapy at Aurinkokartano.
