Iäkkään asukkaan kivunhoito saattohoitovaiheessa

Abstract

Hospice therapy is a symptomatic treatment of a dying person, with the aim of relieving the symptoms of an end-of-life patient. The main goal is to provide the patient with efficient pain relief. With the ageing of the population and the increase in hospice care in homes and home-based service homes, the need for information and knowledge about the pain management of hospice therapy is increasing. By training the nursing and medical staff to maintain their professional skills in hospice therapy, can also benefit the employer and the society.

The purpose of this thesis was to implement an education session for the staff of the residential care home Tervaskoivu about the treatment of pain in elderly people in hospice care. Furthermore, the aim was to use evidence-based data to describe the pain in the elderly in hospice care and the medical and non-pharmaceutical interventions used. The aim of the thesis was furthermore to produce educational materials, which could be used in practical nursing work. In addition, this study promotes the competence of the nursing and medical staff of the commissioner care home and the goal is to ensure quality pain care for the residents of the commissioner care home regardless of the care providing individual.

The thesis focuses on medical pain treatment in terminal care. Small-scale non-pharmaceutical pain treatment, which can be as important as the medical pain treatment, is also discussed.

This is a functional thesis. The process of the thesis included preparation, planning, implementation, concluding, and reporting. These steps included a search for theoretical information, educational planning and implementation, writing the report of the thesis, and analyzing answers to a questionnaire. The educational material was made with the aim to be concise and comprehensive. Based on the feedback received, it can be concluded that the educational material made in this thesis was useful for the commissioner unit and that they received confirmation of their own competence through it. The material produced can potentially also be used in other service units for the elderly and home care. Further work on the material could involve making of a guide or folder for pain management in hospice care.

Saattohoito on kuolevan ihmisen oireenmukaista hoitoa, jonka tarkoituksena on lieventää oireita ja tavoitteena hyvä kivunlievitys. Suuren ikäpolven vanhentuessa ja saattohoidon lisääntyessä kodeissa ja kodinomaisissa asumisyksiköissä, tarve saattohoidon kivunhoidon tiedolle ja osaamiselle lisääntyy. Lisäkoulutuksilla pidetään yllä hoitohenkilöstön ammattitaitoa, samalla siitä hyötyy työnantaja sekä yhteiskunta.

Opinnäytetyömme tarkoituksena oli toteuttaa iäkkään kivunhoito saattohoitovaiheessa -koulutus palvelukoti Tervaskoivun henkilöstölle ja kuvata näyttöön perustuvan tiedon avulla iäkkään saattohoitovaiheen kipua sekä sen lääkkeellistä ja lääkkeetöntä hoitoa. Opinnäytetyömme tavoitteena oli tuottaa materiaali, jota voidaan hyödyntää käytännön hoitotyössä. Toisena tavoitteenamme oli edistää palvelukodin henkilökunnan osaamista saattohoidon kivunhoidossa ja taata hoitokodin asukkaille laadukas kivunhoito viimeisinä hetkinään.

Opinnäytetyön aihe painotettiin lääkkeelliseen kivunhoitoon ikäihmisen saattohoitovaiheessa. Käsittelimme myös pienimuotoisesti lääkkeetöntä kivunhoitoa, joka on yhtä tärkeää kuin lääkkeellinen kivunhoito.

Tuotimme opinnäytetyön toiminnallisena opinnäytetyönä. Opinnäytetyön projektiin kuului valmistelu eli tavoitteen määrittely, suunnittelu, toteuttaminen sekä päättäminen ja arviointi, joihin sisältyi teoriatiedon etsiminen, koulutuksen suunnittelu ja toteutus, opinnäytetyön raportin kirjoittaminen sekä palautekyselyn purkaminen. Koulutusmateriaali oli tiivis ja kattava. Saadun palautteen perusteella voimme päätellä, että yksikössä on koulutusmateriaalille käyttöä ja he saivat vahvistusta omaan osaamiseensa. Materiaali on hyödynnettävissä myös ikäihmisten asumis- ja palveluyksiköissä sekä kotihoidossa. Jatkotyöskentelynä materiaalista voisi toteuttaa saattohoitovaiheen kivunhoidon oppaan tai kansion.
