Healthcare depends on communication and teamwork. Information have to be available for many actors at the same time to make healthcare systems efficient. Availability of information enables quick decisions to be made when needed and also reduces waste by removing repeated work and testing from healthcare system. Also fewer errors are made when information about patient is always in hand. Efficient sharing of health information is possible when different healthcare information systems are interoperable. Interoperability needs to start from bottom of the information flow and go all the way to the top. Many clinical decisions are based on laboratory test results.
Laboratory can help to achieve the interoperability of health data by using uniform nomenclature for test results it produces and standardized messaging of orders and results. There is many different nomenclatures for laboratory tests in use around the world. Internationally most wide spread is LOINC which is used in over 190 countries. In nordic countries NPU nomenclature is used in Sweden, Denmark and Norway. Finland uses nomenclature produced by Kuntaliitto. Finnish laboratories are using this nomenclature widely and data interop-erability is good in Finnish clinical laboratory field. Data transfer in correct form is crucial starting point for laboratory interoperability. Clinical laboratories use standardized messaging format produced by HL7. To have correctly entered and uniform data, analyzer interface can be used. This interface can transfer orders to analyzer and produced result are transferred from the analyzer to LIS. Automated order and results transfer enables error free and uniform messaging.
This thesis gives an insight to interoperability and in little more detail to the clinical laboratory interoperability. Basics of laboratory information system are also discussed. Standardisation of workflow and operations of clinical laboratories are coordinated internationally by IHE. Some of the profiles from IHE concerning laboratory work are introduced and in vitro analyzer interface standard from CLSI is discussed. In the practical part of thesis analyzer interface for Real-time PCR analyzer is established and the data transfer is validated. Interface is now in use in Nordlab Kajaani laboratory.
