Tiivistelmä
Kehittämistyön tarkoitus on ollut perusterveydenhuollon ja erityispalveluiden välisen yhteistyön ja palvelujen välisen integroitumisen tarkastelu käytännön työssä päihdeasiakkaiden hoidossa Oulun kaupungissa. Tutkimuksen tavoitteena on ollut tuottaa päihdetyön toimintamalli Kontinkankaan Hyvinvointikeskukseen ja kehittää päihdeongelmaisten kanssa tehtävää yhteistyötä Kontinkankaan Hyvinvointikeskuksen työntekijöiden sekä mielenterveys- ja päihdepalvelujen työntekijöiden välillä.
Suomessa on globaalin suuntauksen mukaisesti tarve tuottaa sosiaali- ja terveydenhuollon palveluja asiakaslähtöisesti ja moniammatillisesti tulevaisuuden haasteisiin vastaamiseksi. Heikko taloudellinen tilanne edellyttää palvelujärjestelmien muutoksia.
Tutkimukseen osallistui yhteensä kuusitoista hyvinvointikeskuksen, mielenterveyspalvelujen ja päihdepalvelujen sosiaali- ja terveydenhuollon ammattilaista. Tutkimus kohdistui hyvinvointikeskuksen akuutti- ja ajanvarausvastaanottotyöhön sekä mielenterveyspalveluiden ja päihdepalveluiden henkilökunnan kanssa tehtävään yhteistyöhön päihdeasiakkaan hoidossa. Kyseessä on toiminnallinen tutkimus, joka suoritettiin kolmena ryhmähaastatteluna ja yhtenä yksilöhaastatteluna.
Tulosten perusteella perusterveydenhuollon sekä mielenterveys- ja päihdepalvelujen työ on osittain integroitunut päihdeasiakkaan hoidossa. Hyvinvointikeskuksen avauduttua mielenterveyspalveluista siirtyi sinne työskentelemään henkilökuntaa, joka vastaa akuutteihin mielenterveydellisten ongelmien arvioinnin tarpeeseen ja toimii konsultatiivisena tukena hyvinvointikeskuksen henkilökunnalle. Päihdepalvelujen koettiin jääneen syrjään hyvinvointikeskustoiminnasta ja hyvinvointikeskuksen ja mielenterveyspalvelujen henkilökunta toivoi selkeää toimintamallia päihdeasiakkaiden hoitoon ja tietoa päihdepalveluista.
Tulosten mukaan työntekijät tulee osallistaa hyvinvointikeskustyön suunnitteluun ja mahdollistaa työntekijöiden välinen vuoropuhelu. Työtekijöiden työnkuvaa tulee selkeyttää ja päihdetyön osaamista vahvistaa. Mielenterveyspalvelujen työntekijä voi tukea peruster-veydenhuollon työtä päihdeasiakkaan alkuarvioinnissa ja päihdepalvelujen henkilökunta osallistuu jatkohoidon arviointeihin perusterveydenhuollossa ja antaa konsultaatioapua ja osallistuu moniammatillisiin työryhmiin. The purpose of the (development) thesis has been to observe the co-operation and integration between services in the basic health care system and special health care system especially targeting in the care of the substance abusers in the city of Oulu.
Aim of the (development) thesis has been to produce a procedure/formula to substance abuse work to the welfare center in Kontinkangas. Another aim of the thesis has been to develop co-operation between the staff in the welfare center and in the mental health services and substance abuse services in the care of the substance abusers.
The global trend is, which also shows in Finland, to produce social and health care services in a customer oriented approach to respond to the challenges from the future. Also the bad economic situation has an impact on the need to change the system.
16 social care and healthcare professionals working in the welfare center, mental health services and in the substance abuse services took part in this research. The study focused on the welfare centers’ acute health care work and appointment-based health care work and to the collaboration in the care of substance abusers between the staff in welfare center and mental health and substance abuse services. The study is a functional study, which was conducted as three group interviews and one individual interview.
Based on the results of the study, the work of the basic health care system and of the mental health services and substance abuse services is partially integrated in car-ing/treating the substance abusers. After the welfare center was opened, some staff from the mental health services moved to work there. Their job is to respond to the acute as-sessments of mental health problems and work as consultants to the other staff. Sub-stance abuse services were seen to be left aside from the welfare center. Also the staff at the welfare center and at the mental health services hoped to have a clear formula on how to treat substance abusers. They also needed more information about the substance abuse services.
Based on the results of the study, staff should be involved in the planning of the work at the welfare centers and to enable the dialog between workers. The job descriptions must be clarified, and strengthen the knowledge of substance abuse work. Mental healthcare worker can support the work in the basic health care system when assessing a client with substance abuse problems. The staff at the substance abuse services will take part in the follow-up treatment assessment at the welfare center and work as consultant and take part in the multi-professional teams.
Suomessa on globaalin suuntauksen mukaisesti tarve tuottaa sosiaali- ja terveydenhuollon palveluja asiakaslähtöisesti ja moniammatillisesti tulevaisuuden haasteisiin vastaamiseksi. Heikko taloudellinen tilanne edellyttää palvelujärjestelmien muutoksia.
